To by bylo v\u00fdborn\u00e9, d\u011bkuju za tuhle tu pozn\u00e1mku. Tak o <span class=\"highlight\">l\u00e9c\u00edch<\/span>, o kter\u00fdch se to obecn\u011b v\u00ed, bych pohovo\u0159ila jako prvn\u00ed, a to jsou hormony. U kortikoid\u016f je zn\u00e1m\u00e9 to, \u017ee vznik\u00e1 takov\u00fd zvl\u00e1\u0161tn\u00ed typ obezity, takzvan\u00e9ho centripet\u00e1ln\u00edho typu, to znamen\u00e1, \u017ee se zv\u011bt\u0161uje objem b\u0159\u00ed\u0161ka a\u0165 u\u017e u d\u011bt\u00ed nebo u dosp\u011bl\u00fdch. B\u00fdvaj\u00ed takov\u00e9 zarudl\u00e9 tv\u00e1\u0159i\u010dky a strie, \u010dili takov\u00e9 ty pajiz\u00e9vky, kter\u00e9 se tvo\u0159\u00ed v podko\u017e\u00ed, m\u00edvaj\u00ed na nafialov\u011blou barvu. Tady chci \u0159\u00edct, \u017ee k n\u00e1m chod\u00ed spousta lid\u00ed, kte\u0159\u00ed kortikoidy berou z velmi z\u00e1va\u017en\u00fdch d\u016fvod\u016f, nap\u0159\u00edklad pro n\u011bjak\u00e9 autoimunitn\u00ed onemocn\u011bn\u00ed, pro z\u00e1va\u017en\u011b prob\u00edhaj\u00edc\u00ed astmu nebo jako dopln\u011bk l\u00e9\u010dby p\u0159i n\u00e1dorov\u00e9m onemocn\u011bn\u00ed. U t\u011bhle t\u011bch lid\u00ed to zoufalstv\u00ed z toho, \u017ee vlastn\u011b p\u0159ib\u00edraj\u00ed, by m\u011blo b\u00fdt n\u011bjak\u00fdm zp\u016fsobem tro\u0161ku omezeno t\u00edm, \u017ee vlastn\u011b jejich z\u00e1kladn\u00ed choroba se t\u011bmito <span class=\"highlight\">l\u00e9ky<\/span> velmi dob\u0159e l\u00e9\u010d\u00ed a bez nich by prob\u00edhala bolestiv\u011b nebo velmi nep\u0159\u00edzniv\u011b. Kortikoidy maj\u00ed n\u011bkolik negativn\u00edch efekt\u016f. Zvy\u0161uj\u00ed takzvanou inzulinorezistenci, proto\u017ee glukokortikoidy p\u016fsob\u00ed proti inzulinu, proti vlastn\u011b funkci inzulinu, to znamen\u00e1 tk\u00e1n\u011b, zejm\u00e9na i tukov\u00e1 tk\u00e1\u0148, jsou odoln\u00e9 proti odbour\u00e1v\u00e1n\u00ed, a proto ten \u010dlov\u011bk, a\u0165 cvi\u010d\u00ed, necvi\u010d\u00ed, tak prost\u011b nehubne. Dal\u0161\u00ed, vytv\u00e1\u0159\u00ed se v\u011bt\u0161\u00ed mno\u017estv\u00ed viscer\u00e1ln\u00edho tuku, to je ten tuk, kter\u00fd je mezi kli\u010dkami st\u0159evn\u00edmi pod j\u00e1try a v j\u00e1trech, proto se zv\u011bt\u0161uje objem pasu. Je d\u016fle\u017eit\u00e9 tu pohyblivost udr\u017eet. Tak tro\u0161ku antidotuje tenhle negativn\u00ed efekt. Jinak dal\u0161\u00ed, co se d\u011bje, jsou zm\u011bny vylu\u010dov\u00e1n\u00ed pohlavn\u00edch hormon\u016f, doch\u00e1z\u00ed ke ztr\u00e1t\u011b ochlupen\u00ed a dal\u0161\u00ed, co je d\u016fle\u017eit\u00e9 pro v\u0161echny, kte\u0159\u00ed se zaj\u00edmaj\u00ed o zdravou v\u00fd\u017eivu, sn\u00ed\u017e\u00ed se pocit hladu a \u017e\u00edzn\u011b. Tak\u017ee to je moc dob\u0159e, a to, \u017ee se rad\u00edte s <span class=\"highlight\">l\u00e9ka\u0159em<\/span>, je velmi d\u016fle\u017eit\u00e9. Pravd\u011bpodobn\u011b asi pacienti s glaukomem v\u011bd\u00ed, \u017ee ka\u017ed\u00e9mu zdravotn\u00edkovi, s kter\u00fdm p\u0159ijdou do kontaktu, o sv\u00e9m glaukomu mus\u00ed \u0159\u00edct, proto\u017ee to je velk\u00e1 vlastn\u011b p\u0159ek\u00e1\u017eka. My se ale bav\u00edme tady o r\u016fzn\u00fdch efektech vlastn\u011b na hubnut\u00ed. A tady to, \u017ee ty <span class=\"highlight\">l\u00e9ky<\/span> zvy\u0161uj\u00ed chu\u0165 k j\u00eddlu, je velk\u00e1 vlastn\u011b dobr\u00e1 v\u011bc, proto\u017ee ta aktivn\u00ed hmota, kterou takto ztr\u00e1cej\u00ed, se nab\u00edr\u00e1 zp\u00e1tky. Dal\u0161\u00ed <span class=\"highlight\">l\u00e9ky<\/span>, kter\u00e9 zvy\u0161uj\u00ed chu\u0165 k j\u00eddlu, jsou <span class=\"highlight\">l\u00e9ky<\/span>, kter\u00e9 p\u016fsob\u00ed na nervov\u00fd syst\u00e9m, to jsou antiepileptika nebo n\u011bkter\u00e1 neuroleptika. Tady zase je velmi d\u016fle\u017eit\u00e9, aby ten pacient necht\u011bl n\u011bjak v\u00fdrazn\u011b hubnout, proto\u017ee zejm\u00e9na u epilepsie v\u00fdrazn\u00e9 omezov\u00e1n\u00ed tuk\u016f ve stav\u011b a n\u011bjak\u00e9 hektick\u00e9 cvi\u010den\u00ed zejm\u00e9na ve\u010der by naopak mohlo v\u00fdrazn\u011b zhor\u0161it vlastn\u011b progn\u00f3zu t\u00e9 z\u00e1kladn\u00ed choroby, \u010dili epilepsie. Tak\u017ee ur\u010dit\u011b to zv\u00fd\u0161en\u00ed chuti k j\u00eddlu a chuti na tu\u010dn\u00e9 je sp\u00ed\u0161 jako zase bohu\u017eel \u017e\u00e1douc\u00ed tro\u0161ku efekt. Dal\u0161\u00ed <span class=\"highlight\">l\u00e9ky<\/span>, kter\u00e9 maj\u00ed vliv na chu\u0165 k j\u00eddlu, jsou <span class=\"highlight\">l\u00e9ky<\/span> vlastn\u011b, kter\u00e9 zase naopak ji omezuj\u00ed, to je digoxin, kter\u00fd pou\u017e\u00edvaj\u00ed pacienti t\u0159eba se srde\u010dn\u00edm selh\u00e1n\u00edm nebo s n\u011bjakou vrozenou srde\u010dn\u00ed vadou, p\u0159i kter\u00e9 je oslabena funkce stahov\u00e1n\u00ed srd\u00ed\u010dka. N\u011bkter\u00e9 protiz\u00e1n\u011btliv\u00e9 <span class=\"highlight\">l\u00e9ky<\/span>, antiparkinsonika, to je zase v\u00fdznamn\u00e9 pro lidi star\u0161\u00edho v\u011bku. Antibiotika a n\u011bkter\u00e9 cytostatika. Tady bych cht\u011bla \u0159\u00edct, \u017ee snaha o hubnut\u00ed je v\u0161\u00edm t\u00edmhle t\u00edm naru\u0161ena a ten pacient pat\u0159\u00ed bu\u010f do n\u011bjak\u00e9ho programu rehabilita\u010dn\u00edho, kde by bez z\u00e1t\u011b\u017ee kloub\u016f n\u011bjak\u00fdm zp\u016fsobem mohl cvi\u010dit. Mus\u00ed to b\u00fdt ve f\u00e1zi, kdy nem\u00e1 tu z\u00e1n\u011btlivou komplikaci vlastn\u011b vzplanulou, to znamen\u00e1 ve f\u00e1zi klidu. Ale v sou\u010dasnosti tak, jak to \u0159\u00edk\u00e1te, \u017ee bere i opi\u00e1ty, tak se ob\u00e1v\u00e1m, \u017ee nen\u00ed ta vhodn\u00e1 doba na hubnut\u00ed.
