{"id":19600,"date":"2015-01-28T10:43:32","date_gmt":"2015-01-28T09:43:32","guid":{"rendered":"http:\/\/www.invarena.cz\/?p=19600"},"modified":"2015-01-28T10:43:32","modified_gmt":"2015-01-28T09:43:32","slug":"nemocnice-ceske-budejovice-lide-chteji-casto-pracovat-i-za-cenu-poskozeni-zdravi","status":"publish","type":"post","link":"https:\/\/www.invarena.cz\/?p=19600","title":{"rendered":"Nemocnice \u010cesk\u00e9 Bud\u011bjovice: Lid\u00e9 cht\u011bj\u00ed \u010dasto pracovat i za cenu po\u0161kozen\u00ed zdrav\u00ed"},"content":{"rendered":"<p><strong>Pracovn\u00ed l\u00e9ka\u0159i (d\u0159\u00edve z\u00e1vodn\u00ed l\u00e9ka\u0159i), jejich\u017e verdikt rozhoduje o tom, zda \u010dlov\u011bk m\u016f\u017ee vzhledem ke sv\u00e9mu zdravotn\u00edmu stavu nastoupit do zam\u011bstn\u00e1n\u00ed nebo v n\u011bm pokra\u010dovat, se st\u00e1le \u010dast\u011bji st\u00e1vaj\u00ed ter\u010dem verb\u00e1ln\u00edch \u00fatok\u016f ze strany t\u011bch, kter\u00e9 ozna\u010d\u00ed za zdravotn\u011b nezp\u016fsobil\u00e9, \u010di podm\u00edn\u011bn\u011b zdravotn\u011b nezp\u016fsobil\u00e9 pro v\u00fdkon konkr\u00e9tn\u00edho pracovn\u00edho za\u0159azen\u00ed.<\/strong><!--more--><\/p>\n<p>T\u00fdk\u00e1 se to nej\u010dast\u011bji rizikov\u00fdch prac\u00ed, \u010dinnost\u00ed vy\u017eaduj\u00edc\u00ed zvl\u00e1\u0161tn\u00ed zdravotn\u00ed zp\u016fsobilost (nap\u0159. pr\u00e1ce s alergeny, pr\u00e1ce ve v\u00fd\u0161k\u00e1ch, sv\u00e1\u0159e\u010dsk\u00e9 \u010dinnosti a podobn\u011b) a dal\u0161\u00edch speci\u00e1ln\u00edch profes\u00ed, v\u010detn\u011b profesion\u00e1ln\u00edho \u0159\u00edzen\u00ed motorov\u00fdch vozidel. Lid\u00e9 jsou toti\u017e mnohdy ochotni pracovat i za cenu dal\u0161\u00edho po\u0161kozov\u00e1n\u00ed vlastn\u00edho zdrav\u00ed, p\u0159esto\u017ee to legislativa neumo\u017e\u0148uje. P\u0159\u00ed\u010dinou zm\u00edn\u011bn\u00fdch konflikt\u016f jsou podle l\u00e9ka\u0159\u016f p\u0159edev\u0161\u00edm obavy z budoucnosti a soci\u00e1ln\u00ed nejistota, n\u011bkdy ov\u0161em i \u00fa\u010delov\u00e9 jedn\u00e1n\u00ed.<\/p>\n<p>&#8222;Vinu za to, \u017ee d\u00e1le nemohou svoji pr\u00e1ci vykon\u00e1vat, \u010dasto p\u0159en\u00e1\u0161ej\u00ed sm\u011brem k n\u00e1m. N\u011bkdy i velmi nevyb\u00edrav\u00fdm a agresivn\u00edm zp\u016fsobem. Na jedn\u00e9 stran\u011b je to pochopiteln\u00e9, proto\u017ee takov\u00e1 situace p\u0159iv\u00e1d\u00ed tyto lidi do slo\u017eit\u00fdch krizov\u00fdch \u017eivotn\u00edch situac\u00ed. Osobn\u00edch, soci\u00e1ln\u00edch a podobn\u011b. Na druh\u00e9 stran\u011b je to pro n\u00e1s velmi zat\u011b\u017euj\u00edc\u00ed a stresuj\u00edc\u00ed, zvl\u00e1\u0161t\u011b v r\u016fzn\u00fdch spr\u00e1vn\u00edch a soudn\u00edch dohr\u00e1ch,&#8220; uvedl prim\u00e1\u0159 Odd\u011blen\u00ed pracovn\u00edho l\u00e9ka\u0159stv\u00ed Nemocnice \u010cesk\u00e9 Bud\u011bjovice, a. s., MUDr. Ale\u0161 Hejlek.<\/p>\n<p>Ztr\u00e1tou pr\u00e1ce nebo jej\u00edm podm\u00edne\u010dn\u00fdm omezen\u00edm kv\u016fli neodpov\u00eddaj\u00edc\u00edmu zdravotn\u00edmu stavu jsou nejv\u00edce ohro\u017eeni zam\u011bstnanci star\u0161\u00ed 50 let. Nez\u0159\u00eddka se v\u0161ak tento probl\u00e9m t\u00fdk\u00e1 i mlad\u0161\u00edch lid\u00ed, zejm\u00e9na pokud pracuj\u00ed v pracovn\u00edm za\u0159azen\u00ed s rizikov\u00fdmi faktory, v\u010detn\u011b fyzicky n\u00e1ro\u010dn\u00fdch provoz\u016f a ve speci\u00e1ln\u00edch profes\u00edch.<\/p>\n<p>&#8222;Tito lid\u00e9 pak po n\u00e1s cht\u011bj\u00ed, abychom posunuli hranice benevolence posuzov\u00e1n\u00ed zdravotn\u00edho stavu tak, aby mohli ve sv\u00e9 pr\u00e1ci pokra\u010dovat. My ov\u0161em nejen\u017ee nem\u016f\u017eeme p\u0159ekra\u010dovat v t\u00e9to oblasti platn\u00e9 z\u00e1konn\u00e9 normy, ale rozhodn\u011b u\u017e v\u016fbec nesm\u00edme p\u0159ipustit, aby do\u0161lo k dal\u0161\u00edmu zhor\u0161ov\u00e1n\u00ed zdravotn\u00edho stavu kter\u00e9hokoli \u010dlov\u011bka vlivem pr\u00e1ce, pracovn\u00edch podm\u00ednek a prost\u0159ed\u00ed. V tomto sm\u011bru m\u00e1me dan\u00e1 jasn\u00e1 z\u00e1konn\u00e1 i medic\u00ednsk\u00e1 etick\u00e1 pravidla, nav\u00edc by to bylo zcela proti smyslu a \u00fa\u010delu pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eeb,&#8220; konstatoval prim. MUDr. Hejlek.<\/p>\n<p>Od 1. dubna 2012 s nabyt\u00edm \u00fa\u010dinnosti z\u00e1kona \u010d. 373\/2011 Sb., o specifick\u00fdch zdravotn\u00edch slu\u017eb\u00e1ch m\u00e1 ka\u017ed\u00fd zam\u011bstnavatel bez ohledu na po\u010det zam\u011bstnanc\u016f, by\u0165 zam\u011bstn\u00e1v\u00e1 t\u0159eba jednoho, povinnost m\u00edt sv\u00e9ho smluvn\u00edho l\u00e9ka\u0159e pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eeb (d\u0159\u00edve z\u00e1vodn\u00edho l\u00e9ka\u0159e). Zam\u011bstnavatel sou\u010dasn\u011b mus\u00ed zam\u011bstnanc\u016fm sd\u011blit, kter\u00e9 takov\u00e9to smluvn\u00ed za\u0159\u00edzen\u00ed tyto slu\u017eby poskytuje a jak\u00fdm preventivn\u00edm prohl\u00eddk\u00e1m a vy\u0161et\u0159en\u00edm souvisej\u00edc\u00edm s v\u00fdkonem pr\u00e1ce jsou povinni se podrobit. Zam\u011bstnanci mimo jin\u00e9 mus\u00ed b\u00fdt zam\u011bstnavatelem informov\u00e1ni o specifick\u00fdch n\u00e1roc\u00edch sv\u00e9 pr\u00e1ce, rizic\u00edch a mo\u017en\u00e9 ochran\u011b sv\u00e9ho zdrav\u00ed, v\u010detn\u011b ochrann\u00fdch pom\u016fcek. Cel\u00e1 \u0159ada dal\u0161\u00edch povinnost\u00ed zam\u011bstnavatel\u016f a zam\u011bstnanc\u016f v oblasti pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eeb je pops\u00e1na v platn\u00e9m z\u00e1kon\u00edku pr\u00e1ce. Ve\u0161ker\u00e9 smluvn\u00ed pracovn\u011b l\u00e9ka\u0159sk\u00e9 slu\u017eby jsou v pln\u00e9m rozsahu hrazeny zam\u011bstnavatelem a dle z\u00e1kona je sm\u00ed smluvn\u011b vykon\u00e1vat pouze pracovn\u00ed l\u00e9ka\u0159i a prakti\u010dt\u00ed l\u00e9ka\u0159i. Nikdo jin\u00fd.<\/p>\n<p>&#8222;Z\u00e1kladn\u00ed filozofie pracovn\u011bl\u00e9ka\u0159sk\u00e9 p\u00e9\u010de, kter\u00e1 m\u00e1 preventivn\u00ed charakter, spo\u010d\u00edv\u00e1 v tom, \u017ee zam\u011bstnanec m\u00e1 vzhledem ke sv\u00e9mu sou\u010dasn\u00e9mu i dlouhodob\u00e9mu zdravotn\u00edmu stavu vykon\u00e1vat takov\u00fd druh pr\u00e1ce, u n\u00ed\u017e je pravd\u011bpodobnost, \u017ee ho nebude zdravotn\u011b po\u0161kozovat a z\u00e1rove\u0148 bude v\u0161echny aspekty pracovn\u00edho za\u0159azen\u00ed zdravotn\u011b zvl\u00e1dat. A\u0165 u\u017e v tom smyslu, \u017ee by na dan\u00e9m pracovi\u0161ti po\u0161kozen\u00ed vzniklo, tak i v tom smyslu, \u017ee posuzovan\u00fd \u010dlov\u011bk je ji\u017e n\u011bjak\u00fdm zp\u016fsobem nemocn\u00fd a p\u0159i konkr\u00e9tn\u00ed pr\u00e1ci by se jeho zdravotn\u00ed obt\u00ed\u017ee d\u00e1le zhor\u0161ily,&#8220; poznamenal prim. MUDr. Hejlek.<\/p>\n<p>Nejv\u00edce jsou vlivem pr\u00e1ce na sv\u00e9 zdrav\u00ed zat\u00ed\u017eeni samoz\u0159ejm\u011b lid\u00e9 v t\u011b\u017ek\u00fdch provozech s \u010dast\u00fdmi rizikov\u00fdmi faktory a kombinovan\u00fdmi riziky. Z\u00e1v\u011br l\u00e9ka\u0159e z pracovn\u011bl\u00e9ka\u0159sk\u00e9 prohl\u00eddky, tedy takzvan\u00e1 zdravotn\u00ed zp\u016fsobilost k pr\u00e1ci, toti\u017e vych\u00e1z\u00ed v\u017edy z kombinace posouzen\u00ed n\u00e1ro\u010dnosti pr\u00e1ce a zdravotn\u00edho stavu pracovn\u00edka. Mus\u00ed se j\u00edm \u0159\u00eddit zam\u011bstnavatel i zam\u011bstnanec.<\/p>\n<p>&#8222;Nejv\u011bt\u0161\u00ed probl\u00e9my vznikaj\u00ed v situac\u00edch, kdy k n\u00e1m p\u0159ijde \u010dlov\u011bk na vy\u0161et\u0159en\u00ed v r\u00e1mci pracovn\u011bl\u00e9ka\u0159sk\u00e9 prohl\u00eddky ji\u017e s ur\u010dit\u00fdm zdravotn\u00edm po\u0161kozen\u00edm, je\u017e ho limituje pro posuzovanou pracovn\u00ed \u010dinnost, st\u00e1v\u00e1 se zdravotn\u011b nezp\u016fsobil\u00fdm, nebo podm\u00edne\u010dn\u011b zp\u016fsobil\u00fdm, \u010dili nem\u016f\u017ee d\u00e1le vykon\u00e1vat \u010dinnost, pro kterou je zam\u011bstn\u00e1n anebo nikoliv v pln\u00e9m jej\u00edm rozsahu. Je\u0161t\u011b ov\u0161em nedosahuje takov\u00e9ho stupn\u011b po\u0161kozen\u00ed, abychom mohli uva\u017eovat o nemoci z povol\u00e1n\u00ed, nato\u017e ji ohl\u00e1sit. P\u0159itom ji\u017e m\u00e1 \u010d\u00e1ste\u010dn\u011b po\u0161kozen\u00e9 zdrav\u00ed z pr\u00e1ce a t\u00e9m\u011b\u0159 \u017e\u00e1dn\u00e9 soci\u00e1ln\u00ed n\u00e1roky. Pr\u00e1v\u011b t\u011bmto lidem nejv\u00edce hroz\u00ed n\u00e1sledn\u00e1 dlouhodob\u00e1 nezam\u011bstnanost,&#8220; zd\u016fraznil prim\u00e1\u0159 Hejlek.<\/p>\n<p>V podobn\u00fdch p\u0159\u00edpadech se pak l\u00e9ka\u0159i sna\u017e\u00ed pomoci sn\u00ed\u017eit negativn\u00ed soci\u00e1ln\u00ed dopady posti\u017een\u00fdch pracovn\u00edk\u016f. &#8222;P\u0159ed p\u00e1r lety jsme v r\u00e1mci smluvn\u00edch pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eeb p\u0159evzali podnik, kde bylo na zdravotn\u00edm stavu zam\u011bstnanc\u016f z\u0159eteln\u011b zn\u00e1t dlouholet\u00e9 velmi n\u00e1ro\u010dn\u00e9 pracovn\u00ed vyt\u00ed\u017een\u00ed. Nejv\u00edce ve v\u011bku mezi pades\u00e1tkou a \u0161edes\u00e1tkou. \u010c\u00e1st z nich v\u016fbec, nebo \u010d\u00e1ste\u010dn\u011b, nespl\u0148ovala zdravotn\u00ed zp\u016fsobilost k posuzovan\u00e9 pr\u00e1ci. Jim se to samoz\u0159ejm\u011b, stejn\u011b jako podniku, nel\u00edbilo, byli to zku\u0161en\u00ed a t\u011b\u017eko postradateln\u00ed lid\u00e9. Jednali jsme ka\u017edou chv\u00edli s veden\u00edm, hledali mo\u017enosti jejich uplatn\u011bn\u00ed, p\u0159imlouvali se. Nebylo to jednoduch\u00e9. Sna\u017e\u00edme se, co m\u016f\u017eeme, pro n\u00e1s z\u016fst\u00e1v\u00e1 \u010dlov\u011bk \u010dlov\u011bkem nikoli jen jm\u00e9nem na seznamu. Nepracujeme od stolu. Na n\u011bkter\u00e9 syst\u00e9mov\u00e9 legislativn\u00ed zm\u011bny, kter\u00e9 funguj\u00ed ve vysp\u011bl\u00e9 Evrop\u011b, jsme ov\u0161em kr\u00e1tc\u00ed, i kdy\u017e ur\u010dit\u00e9 v\u00fdhledy se r\u00fdsuj\u00ed,&#8220; doplnil prim\u00e1\u0159.<\/p>\n<p>Odd\u011blen\u00ed pracovn\u00edho l\u00e9ka\u0159stv\u00ed Nemocnice \u010cesk\u00e9 Bud\u011bjovice, a. s., m\u00e1 velkou ambulantn\u00ed \u010d\u00e1st a vyu\u017e\u00edv\u00e1 n\u011bkolika l\u016f\u017eek na intern\u00edm odd\u011blen\u00ed. Jako jedin\u00e9 pracovi\u0161t\u011b v Jiho\u010desk\u00e9m kraji a na Pelh\u0159imovsku poskytuje komplexn\u00ed specializovanou pracovn\u011b-l\u00e9ka\u0159skou p\u00e9\u010di p\u0159edev\u0161\u00edm v oblasti klinick\u00e9ho pracovn\u00edho l\u00e9ka\u0159stv\u00ed.<\/p>\n<p>Jako z\u00e1konem uznan\u00e9 region\u00e1ln\u00ed st\u0159edisko nemoc\u00ed z povol\u00e1n\u00ed zaji\u0161\u0165uje \u0161et\u0159en\u00ed a hl\u00e1\u0161en\u00ed profesion\u00e1ln\u00edch onemocn\u011bn\u00ed. Metodicky vede ambulanci pro profesion\u00e1ln\u00ed ko\u017en\u00ed onemocn\u011bn\u00ed. Sou\u010d\u00e1st\u00ed jeho slu\u017eeb je \u0161et\u0159en\u00ed, hl\u00e1\u0161en\u00ed, dispenzarizace, l\u00e9\u010dba i prevence profesion\u00e1ln\u00edch chorob.<\/p>\n<p>Pracovi\u0161t\u011b konzili\u00e1rn\u011b vy\u0161et\u0159uje zdravotn\u00ed zp\u016fsobilost ob\u010dan\u016f k pr\u00e1ci, k pracovn\u00edmu za\u0159azen\u00ed, p\u0159edev\u0161\u00edm na rizikov\u00e1 pracovi\u0161t\u011b. Nab\u00edz\u00ed slu\u017eby z\u00e1kladn\u00ed pracovn\u011bl\u00e9ka\u0159sk\u00e9 p\u00e9\u010de, kdy se v r\u00e1mci legislativn\u011b povinn\u00fdch &#8222;smluv o pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eb\u00e1ch&#8220; se zam\u011bstnavateli, komplexn\u011b star\u00e1 o pracovn\u00edky hlavn\u011b z rizikov\u00fdch, ale i nerizikov\u00fdch provoz\u016f. Velkou v\u00fdhodou je mo\u017enost prov\u00e1d\u011bt p\u0159i prohl\u00eddce v\u011bt\u0161inu vy\u0161et\u0159en\u00ed v jednom dni p\u0159\u00edmo na odd\u011blen\u00ed. Tyto komplexn\u00ed slu\u017eby jsou spojen\u00e9 s dohledem na pracovi\u0161ti a s poradenskou \u010dinnost\u00ed. Firm\u00e1m i jednotlivc\u016fm nab\u00edz\u00ed komer\u010dn\u00ed zdravotn\u00ed prohl\u00eddky, mana\u017eersk\u00e1 vy\u0161et\u0159en\u00ed apod. Sou\u010d\u00e1st\u00ed odd\u011blen\u00ed je specializovan\u00e1 ambulance pro pracovn\u00edky vyj\u00ed\u017ed\u011bj\u00edc\u00ed do zahrani\u010d\u00ed, kter\u00e1 se zab\u00fdv\u00e1 jejich pracovn\u00ed zp\u016fsobilost\u00ed.<\/p>\n<p>Odd\u011blen\u00ed pracovn\u00edho l\u00e9ka\u0159stv\u00ed poskytuje v p\u0159idru\u017een\u00e9 ambulanci pracovn\u011bl\u00e9ka\u0159sk\u00fdch slu\u017eeb pro nemocnici tak\u00e9 slu\u017eby praktick\u00e9ho l\u00e9ka\u0159e pro dosp\u011bl\u00e9, a to jak vlastn\u00edm zam\u011bstnanc\u016fm nemocnice, tak i registrovan\u00fdm pacient\u016fm z \u0159ad \u0161irok\u00e9 ve\u0159ejnosti.<\/p>\n<p>Nemocnice \u010cesk\u00e9 Bud\u011bjovice, a. s., pat\u0159\u00ed se svou l\u016f\u017ekovou kapacitou 1450 l\u016f\u017eek mezi nejv\u011bt\u0161\u00ed zdravotnick\u00e1 za\u0159\u00edzen\u00ed v \u010cesk\u00e9 republice. Poskytuje z\u00e1kladn\u00ed, specializovanou i vysoce specializovanou p\u00e9\u010di prakticky ve v\u0161ech medic\u00ednsk\u00fdch oborech zejm\u00e9na pro cel\u00fd jiho\u010desk\u00fd region.<\/p>\n<p>V Nemocnici \u010cesk\u00e9 Bud\u011bjovice, jej\u00edm\u017e jedin\u00fdm akcion\u00e1\u0159em je Jiho\u010desk\u00fd kraj, je ro\u010dn\u011b hospitalizov\u00e1no okolo 53 tis\u00edc pacient\u016f.
