Tak snadn\u00e1 je prevence a n\u011bkdy i l\u00e9\u010dba onemocn\u011bn\u00ed, kter\u00e9 se st\u00e1v\u00e1 celosv\u011btovou hrozbou. V\u00fdsledky v\u016fbec prvn\u00ed studie Sv\u011btov\u00e9 zdravotnick\u00e9 organizace (WHO) zam\u011b\u0159en\u00e9 na v\u00fdskyt diabetu na cel\u00e9 zem\u011bkouli jsou alarmuj\u00edc\u00ed.<\/strong><br \/>\n<!--more--><\/p>\n<p> Za posledn\u00edch 30 let tohoto onemocn\u011bn\u00ed p\u0159ibylo \u010dty\u0159n\u00e1sobn\u011b. Jestli\u017ee se cukrovka, zejm\u00e9na 2. typu, d\u0159\u00edve vyskytovala p\u0159ev\u00e1\u017en\u011b u star\u0161\u00edch lid\u00ed, dnes nen\u00ed \u017e\u00e1dnou v\u00fdjimkou u dosp\u00edvaj\u00edc\u00edch a u d\u011bt\u00ed. Tuto epidemii je p\u0159itom podle odborn\u00edk\u016f mo\u017eno zastavit. &#8222;Onemocn\u011bn\u00edm u n\u00e1s trp\u00ed ji\u017e t\u00e9m\u011b\u0159 deset procent populace. P\u0159itom 80 procent\u016fm diabetu 2. typu lze p\u0159edej\u00edt,&#8220; zd\u016fraz\u0148uje Alena \u0160teflov\u00e1 z kancel\u00e1\u0159e Sv\u011btov\u00e9 zdravotnick\u00e9 organizace v Praze.<\/p>\n<p>Pr\u00e1v\u011b WHO ve sv\u00e9 nejnov\u011bj\u0161\u00ed zpr\u00e1v\u011b varuje, \u017ee pokud se p\u0159\u00edstup k t\u00e9to chorob\u011b radik\u00e1ln\u011b nezm\u011bn\u00ed, bude v roce 2030 sedmou nej\u010dast\u011bj\u0161\u00ed p\u0159\u00ed\u010dinou smrti. Podle \u0161\u00e9fky WHO Margaret Chanov\u00e9 ji\u017e neplat\u00ed, \u017ee diabetes je probl\u00e9m p\u0159edev\u0161\u00edm bohat\u00fdch zem\u00ed. S touto z\u00e1va\u017enou poruchou metabolismu se st\u00e1le v\u00edce pot\u00fdkaj\u00ed i st\u00e1ty se st\u0159edn\u00edmi a n\u00edzk\u00fdmi p\u0159\u00edjmy.<\/p>\n<p>&#8222;Pokud chceme u\u010dinit n\u011bjak\u00fd pokrok v zastaven\u00ed vzestupu cukrovky, mus\u00edme zv\u00e1\u017eit na\u0161e ka\u017edodenn\u00ed \u017eivoty \u2013 j\u00edst zdrav\u011b, b\u00fdt fyzicky aktivn\u00ed a vyvarovat se nep\u0159im\u011b\u0159en\u00e9ho n\u00e1r\u016fstu t\u011blesn\u00e9 hmotnosti,&#8220; \u0159\u00edk\u00e1 Chanov\u00e1. Odhaduje se, \u017ee za 17 procent v\u0161ech \u00famrt\u00ed je odpov\u011bdn\u00fd pr\u00e1v\u011b vysok\u00fd BMI (index t\u011blesn\u00e9 hmotnosti). V evropsk\u00e9m regionu trp\u00ed nadv\u00e1hou t\u00e9m\u011b\u0159 63 procent mu\u017e\u016fa 55 procent \u017een.<\/p>\n<p>&nbsp;<\/p>\n<p>Z\u00e1zra\u010dn\u00fd pohyb<\/p>\n<p>&nbsp;<\/p>\n<p>Jak uk\u00e1zaly posledn\u00ed studie proveden\u00e9 nez\u00e1visle v r\u016fzn\u00fdch st\u00e1tech, fyzick\u00e1 aktivita m\u00e1 nejen na v\u00fdskyt, ale i na samotnou l\u00e9\u010dbu onemocn\u011bn\u00ed mnohem z\u00e1sadn\u011bj\u0161\u00ed vliv, ne\u017e se myslelo. &#8222;Ukazuje se, \u017ee pohyb je zcela nejv\u00fdznamn\u011bj\u0161\u00edm opat\u0159en\u00edm v prevenci diabetu. P\u0159id\u00e1\u00adli diabetik ke sv\u00e9 denn\u00ed aktivit\u011b jen tis\u00edc krok\u016f nav\u00edc, sn\u00ed\u017e\u00ed se hladina cukru po j\u00eddle o 1,2 mmol\/l, co\u017e p\u0159edstavuje dvojn\u00e1sobek poklesu, kter\u00e9ho lze dos\u00e1hnout dnes pou\u017e\u00edvan\u00fdm l\u00e9kem,&#8220; dokl\u00e1d\u00e1 v\u00fdznam pohybu p\u0159ednosta 3. Intern\u00ed kliniky 1. LF UK a VFN v Praze \u0160t\u011bp\u00e1n Sva\u010dina.<\/p>\n<p>D\u016fle\u017eit\u00e9 je i spr\u00e1vn\u00e9 stravov\u00e1n\u00ed. Jak p\u0159ipom\u00edn\u00e1 Sva\u010dina, diabetes nen\u00ed nemoc z p\u0159\u00edjmu cukru, jak se st\u00e1le mnoz\u00ed domn\u00edvaj\u00ed. Je vyvol\u00e1na p\u0159edev\u0161\u00edm p\u0159\u00edjmem \u017eivo\u010di\u0161n\u00fdch tuk\u016f. Podle pr\u016fzkumu agentury STEM\/MARK ze za\u010d\u00e1tku tohoto roku p\u0159itom t\u00e9m\u011b\u0159 polovina dot\u00e1zan\u00fdch nej\u00ed v\u016fbec ovoce a zeleninu, kde je obsa\u017een v\u00fdznamn\u00fd pod\u00edl pot\u0159ebn\u00e9 vl\u00e1kniny a vitamin\u016f. Cel\u00fdch 30 procent pak nedodr\u017euje pitn\u00fd re\u017eim.<\/p>\n<p>&#8222;Zdrav\u00e9 stravov\u00e1n\u00ed m\u016f\u017ee pomoci p\u0159edch\u00e1zet cel\u00e9 \u0159ad\u011b z\u00e1va\u017en\u00fdch onemocn\u011bn\u00ed, jako je obezita nebo cukrovka. Naopak se \u0161patn\u00fdm j\u00eddeln\u00ed\u010dkem mohou b\u00fdt spojeny n\u011bkter\u00e9 typy n\u00e1dorov\u00fdch onemocn\u011bn\u00ed,&#8220; p\u0159ipom\u00edn\u00e1 Kate\u0159ina \u0160imkov\u00e1, nutri\u010dn\u00ed specialistka a odborn\u00e1 garantka spole\u010dnosti Zdrav\u00e9 stravov\u00e1n\u00ed.<\/p>\n<p>&#8222;Z\u00e1sadn\u00ed v\u00fdznam m\u00e1 kombinace pohybov\u00e9 aktivity a vhodn\u00e9ho dietn\u00edho re\u017eimu. Samotn\u00e9 u\u017e\u00edv\u00e1n\u00ed i velmi \u00fa\u010dinn\u00fdch l\u00e9k\u016f bez aktivn\u00ed \u00fa\u010dasti pacienta nesta\u010d\u00ed,&#8220; zd\u016fraz\u0148uje m\u00edstop\u0159edseda \u010desk\u00e9 diabetologick\u00e9 spole\u010dnosti \u010cLS JEP Jan \u0160krha. Jak se ale ukazuje, aktivn\u00ed p\u0159\u00edstup pacient\u016f je v \u010cesku st\u00e1le je\u0161t\u011b probl\u00e9m.<\/p>\n<p>&nbsp;<\/p>\n<p>Ne\u017e p\u0159ijde zabij\u00e1k<\/p>\n<p>&nbsp;<\/p>\n<p>Novou kampa\u0148 pro lidi s prediabetem, tedy stavem, kdy \u010dlov\u011bk m\u00e1 hrani\u010dn\u00ed hladinu cukru v krvi (5,6 \u2013 6,9 mmol\/l), spustili v posledn\u00edch dnech v USA, ale i Velk\u00e9 Brit\u00e1nii \u010di \u0160pan\u011blsku. Jen v Americe je 86 milion\u016f lid\u00ed s hrani\u010dn\u00ed hodnotou cukru, p\u0159i\u010dem\u017e 30 procent z nich do p\u011bti let cukrovkou onemocn\u00ed.<\/p>\n<p>V \u010cesku je s prediabetem, o n\u011bm\u017e se je\u0161t\u011b do ned\u00e1vna p\u0159\u00edli\u0161 nemluvilo, podchyceno p\u011bt procent populace. Odhaduje se ale, \u017ee a\u017e 90 procent lid\u00ed, kte\u0159\u00ed j\u00edm trp\u00ed, o tom v\u016fbec nev\u00ed. Tento stav nejen\u017ee zvy\u0161uje riziko cukrovky 2. typu, ale znamen\u00e1 i v\u011bt\u0161\u00ed pravd\u011bpodobnost v\u00fdskytu kardiovaskul\u00e1rn\u00edch chorob a rakoviny. Pr\u00e1v\u011b v t\u00e9to f\u00e1zi lze p\u0159itom cukrovce nejsn\u00e1ze a nej\u00fa\u010dinn\u011bji p\u0159edej\u00edt.<\/p>\n<p>\u010cesk\u00fdm pacient\u016fm krom\u011b informac\u00ed \u010dasto chyb\u00ed uk\u00e1zn\u011bnost. St\u00e1le je hodn\u011b t\u011bch, kte\u0159\u00ed l\u00e9ka\u0159sk\u00fdch rad p\u0159\u00edli\u0161 nedbaj\u00ed nebo je \u00fapln\u011b ignoruj\u00ed. Chlubit se nem\u016f\u017eeme ani s p\u0159\u00edstupem k prevenci. Tu st\u00e1le \u0159ada z n\u00e1s podce\u0148uje. &#8222;Bezplatnou prevenci prov\u00e1d\u00ed praktick\u00fd l\u00e9ka\u0159 v 18 letech a n\u00e1sledn\u011b pak po 40. roce v\u011bku ve dvoulet\u00fdch intervalech. P\u0159i nich je zji\u0161\u0165ov\u00e1na glyk\u00e9mie (hladiny cukru v krvi),&#8220; p\u0159ipom\u00edn\u00e1 mluv\u010d\u00ed VZP Old\u0159ich Tich\u00fd. Nav\u00edc \u0159ada poji\u0161\u0165oven dnes v r\u00e1mci sv\u00fdch bonusov\u00fdch program\u016f nab\u00edz\u00ed i p\u0159\u00edsp\u011bvek na \u0159adu pohybov\u00fdch aktivit zam\u011b\u0159en\u00fdch na zlep\u0161en\u00ed zdravotn\u00edho stavu.<\/p>\n<p>A\u010dkoli se \u010desk\u00e1 vl\u00e1da v r\u00e1mci programu N\u00e1rodn\u00ed strategie Zdrav\u00ed 2020 t\u00e9to problematice v\u011bnuje, podstatn\u00e1 \u010d\u00e1st z\u016fst\u00e1v\u00e1 na l\u00e9ka\u0159\u00edch. P\u0159\u00edkladem je pr\u00e1ce 3. intern\u00ed kliniky 1. LF UK, kter\u00e1 rozv\u00edj\u00ed vzd\u011bl\u00e1vac\u00ed projekt pro pacienty s diabetem 1. i 2. typu. Ten je zam\u011b\u0159en na ty, kte\u0159\u00ed maj\u00ed z\u00e1jem z\u00e1sadn\u00ed a aktivn\u00ed zm\u011bnou \u017eivotn\u00edho stylu zlep\u0161it sv\u016fj zdravotn\u00ed stav. Sou\u010d\u00e1st\u00ed je nap\u0159\u00edklad pravideln\u00e1 pr\u00e1ce s glukometrem, krokom\u011brem \u010di zvl\u00e1dnut\u00ed syst\u00e9mu pro nep\u0159etr\u017eit\u00e9 sledov\u00e1n\u00ed hladiny cukru v b\u011b\u017en\u00fdch i rizikov\u00fdch \u017eivotn\u00edch situac\u00edch. Tato eduka\u010dn\u00ed \u010dinnost je jak individu\u00e1ln\u00ed, tak skupinov\u00e1 a jej\u00ed sou\u010d\u00e1st\u00ed jsou dvakr\u00e1t do roka \u010dty\u0159denn\u00ed \u0161kolen\u00ed.<\/p>\n<p>Podle WHO by z\u00e1kladn\u00edm krokem k n\u00e1prav\u011b ze strany jednotliv\u00fdch st\u00e1t\u016f m\u011blo b\u00fdt lep\u0161\u00ed urbanistick\u00e9 \u0159e\u0161en\u00ed m\u011bst tak, aby se v\u00edce po\u010d\u00edtalo s cyklisty a chodci. Odborn\u00edci apeluj\u00ed na potravin\u00e1\u0159sk\u00fd pr\u016fmysl, aby omezil cukry a tuky ve sv\u00fdch v\u00fdrobc\u00edch. U diabetik\u016f 2. typu se vedle zm\u011bny \u017eivotn\u00edho stylu oproti minulosti ihned v okam\u017eiku diagn\u00f3zy nasazuje i farmakologick\u00e1 l\u00e9\u010dba. Pod\u00edl pacient\u016f l\u00e9\u010den\u00fdch pouze dietou v roce 2013 klesl na 13 procent. Je\u0161t\u011b v roce 2004 to bylo skoro 31 procent.<\/p>\n<p>&nbsp;<\/p>\n<p>Za posledn\u00ed t\u0159i roky se prudce zv\u00fd\u0161il po\u010det lid\u00ed l\u00e9\u010den\u00fdch modern\u00edmi l\u00e9ky, tzv gliptiny. I d\u00edky nim je dnes riziko \u00famrt\u00ed u pacient\u016f ve v\u011bku mezi 50 a 70 lety t\u00e9m\u011b\u0159 stejn\u00e9 jako u b\u011b\u017en\u00e9 populace.<\/p>\n<p>&nbsp;<\/p>\n<p>D\u011bl\u00ed se na dva typy podle p\u0159\u00ed\u010diny vzniku. Zat\u00edmco diabetes 1. typu vznik\u00e1 v d\u016fsledku nep\u0159\u00edtomnosti inzul\u00ednu v t\u011ble, k diabetu 2. typu doch\u00e1z\u00ed v p\u0159\u00edpad\u011b, kdy t\u011blo neum\u00ed s inzul\u00ednem, hormonem kontroluj\u00edc\u00edm p\u0159em\u011bnu cukr\u016f v t\u011ble, zach\u00e1zet.<\/p>\n<p>&nbsp;<\/p>\n<p>Kdybychom od sv\u00fdch osmn\u00e1cti let u\u0161li ka\u017ed\u00fd den 10 000 krok\u016f, vyhnuli bychom se v\u0161em chronick\u00fdm onemocn\u011bn\u00edm v\u010detn\u011b cukrovky<\/p>\n<p>&nbsp;<\/p>\n<p>V roce 2014 j\u00ed na sv\u011bt\u011b trp\u011blo 422 milion\u016f dosp\u011bl\u00fdch, tedy ka\u017ed\u00fd jeden\u00e1ct\u00fd (to je o 314 milion\u016f v\u00edce ne\u017e v roce 1980) 60 milion\u016f Evropan\u016f 3,7 milionu lid\u00ed ro\u010dn\u011b zem\u0159e n\u00e1sledkem vysok\u00e9 hladiny cukru v krvi (43 % z nich p\u0159ed 70. rokem v\u011bku)<\/p>\n<p>&nbsp;<\/p>\n<p>P\u0159i nedodr\u017eov\u00e1n\u00ed pravidel hroz\u00ed (po\u010det p\u0159\u00edpad\u016f v \u010cR 2014) ? vznik z\u00e1va\u017en\u00fdch chorob srdce a c\u00e9v \u2013 riziko infarktu 5x \u010dast\u011bj\u0161\u00ed ? po\u0161kozen\u00ed a\u017e selh\u00e1n\u00ed ledvin (97 100) ? po\u0161kozen\u00ed o\u010dn\u00ed s\u00edtnice (100 600), vedouc\u00ed k oslepnut\u00ed (2200) ? syndrom diabetick\u00e9 nohy (40 000) ? amputace doln\u00edch kon\u010detin (10 000) ? sn\u00ed\u017een\u00ed obranyschopnosti a z toho plynouc\u00ed \u010dast\u011bj\u0161\u00ed infek\u010dn\u00ed nemoci ? po\u0161kozen\u00ed nerv\u016f v cel\u00e9m t\u011ble ? n\u00e1r\u016fst rizika onkologick\u00fdch onemocn\u011bn\u00ed<\/p>\n<p>&nbsp;<\/p>\n<p>? komplikace v t\u011bhotenstv\u00ed ? zv\u00fd\u0161en\u00ed rizika ka\u017een\u00ed a vypad\u00e1v\u00e1n\u00ed zub\u016f a vznik z\u00e1n\u011bt\u016f a infekc\u00ed ? zap\u0159\u00ed\u010di\u0148uje dv\u011b procenta \u00famrt\u00ed<\/p>\n<p>&nbsp;<\/p>\n<p>Um\u011bl\u00e1 slinivka Je to tenou\u010dk\u00e1 polymerov\u00e1 desti\u010dka o velikosti c\u00e9d\u00e9\u010dka, kter\u00e1 se implantuje do b\u0159icha. Um\u011bl\u00e1 slinivka b\u0159i\u0161n\u00ed s biologick\u00fdmi prvky, vyvinut\u00e1 francouzsk\u00fdmi v\u011bdci, se podle agentury AFP na \u010dlov\u011bku testuje od za\u010d\u00e1tku tohoto roku. Pacienti si u\u017e nebudou muset p\u00edchat injekce inzulinu: hormon budou p\u0159irozenou cestou produkovat bu\u0148ky slinivky b\u0159i\u0161n\u00ed z\u00edskan\u00e9 genetick\u00fdm in\u017een\u00fdrstv\u00edm nebo z kmenov\u00fdch bun\u011bk.
