Z bakal\u00e1\u0159sk\u00e9 pr\u00e1ce absolventky oboru nutri\u010dn\u00ed terapie na 1. Z novin a \u010dasopis\u016f sice lid\u00e9 na\u010derpaj\u00ed mnoho informac\u00ed, ale sp\u00ed\u0161e p\u0159eva\u017euj\u00ed \u010dl\u00e1nky o nespr\u00e1vn\u00fdch diet\u00e1ch, nav\u00edc \u010dasto spojen\u00e9 s reklamou na potravinov\u00e9 dopl\u0148ky.<\/p>\n<p>\u201eNa z\u00e1kladn\u00edch \u0161kol\u00e1ch se sice vyu\u010duje p\u0159edm\u011bt V\u00fdchova ke spr\u00e1vn\u00e9mu zdravotn\u00edmu stylu, ale \u017e\u00e1ci se nic nedov\u011bd\u00ed nap\u0159\u00edklad o prevenci ateroskler\u00f3zy, cukrovky \u010di n\u00e1dorov\u00fdch onemocn\u011bn\u00ed,\u201c \u0159\u00edk\u00e1 prof. MUDr. \u0160t\u011bp\u00e1n Sva\u010dina, DrSc., p\u0159ednosta III. intern\u00ed kliniky 1. LF UK a V\u0161eobecn\u00e9 fakultn\u00ed nemocnice v Praze a p\u0159edseda \u010cLS JEP. L\u00e9pe na tom nejsou ani studenti st\u0159edn\u00edch \u0161kol.<\/p>\n<p>\u201eN\u00e1\u0161 n\u00e1rod vykazuje ve srovn\u00e1n\u00ed s ostatn\u00edmi evropsk\u00fdmi zem\u011bmi relativn\u011b malou znalost spojitost\u00ed mezi \u017eivotn\u00edm stylem, zejm\u00e9na v\u00fd\u017eivou, pohybovou aktivitou a posilov\u00e1n\u00edm zdrav\u00ed. Potraviny a n\u00e1poje bohat\u00e9 cukry, \u017eivo\u010di\u0161n\u00fdmi tuky nebo sol\u00ed vytv\u00e1\u0159ej\u00ed biologickou z\u00e1vislost, proto\u017ee n\u00e1m chutnaj\u00ed a zp\u016fsobuj\u00ed pocity pohody. Aby tato pohoda byla stabiln\u00ed, pot\u0159ebujeme je st\u00e1le ve v\u011bt\u0161\u00ed m\u00ed\u0159e,\u201c uv\u00e1d\u00ed doc. MUDr. Dana M\u00fcllerov\u00e1, Ph.D., prod\u011bkanka LF UK v Plzni a vedouc\u00ed \u00dastavu hygieny a preventivn\u00ed medic\u00edny LF UK v Plzni.<\/p>\n<p>Prod\u011bkanka dopl\u0148uje, \u017ee na druh\u00e9 stran\u011b je ji\u017e dostate\u010dn\u00e1 b\u00e1ze znalost\u00ed podle medic\u00edny zalo\u017een\u00e9 na d\u016fkazech, \u017ee jejich nadbytek v na\u0161\u00ed strav\u011b, zvl\u00e1\u0161t\u011b v situac\u00edch, kdy nen\u00ed dostate\u010dn\u00fd pohyb, je rizikov\u00fd pro zdrav\u00ed.<\/p>\n<p>Upozor\u0148uje, \u017ee p\u0159ib\u00fdv\u00e1 d\u011bt\u00ed a hlavn\u011b dosp\u00edvaj\u00edc\u00edch a dosp\u011bl\u00fdch s obezitou a jej\u00edmi komplikacemi, jako jsou cukrovka, vysok\u00fd tlak a \u0159ada dal\u0161\u00edch, kter\u00e9 vedou ke sn\u00ed\u017een\u00ed kvality i d\u00e9lky \u017eivota.<\/p>\n<p>\u201eProto \u0159ada zem\u00ed p\u0159istupuje k c\u00edlen\u00e9 politice reformulace potravin tak, aby ve spolupr\u00e1ci s potravin\u00e1\u0159sk\u00fdm pr\u016fmyslem doch\u00e1zelo k ozdrav\u011bn\u00ed v\u00fd\u017eivy ve smyslu zlep\u0161en\u00e9 nab\u00eddky potravin se sn\u00ed\u017een\u00fdm obsahem soli, cukru a \u017eivo\u010di\u0161n\u00fdch tuk\u016f. Chceme-li se sna\u017eit o dobr\u00e9 zdrav\u00ed n\u00e1roda, pak nen\u00ed mo\u017en\u00fd jin\u00fd trend,\u201c dod\u00e1v\u00e1 doc. M\u00fcllerov\u00e1.<\/p>\n<p>Co je vlastn\u011b zdrav\u00e1 v\u00fd\u017eiva?<\/p>\n<p>Potraviny lze rozd\u011blit na obecn\u011b v\u00edce a m\u00e9n\u011b zdrav\u00ed prosp\u011b\u0161n\u011bj\u0161\u00ed. V z\u00e1sad\u011b plat\u00ed, \u017ee zdrav\u00e1 strava se m\u00e1 skl\u00e1dat p\u0159edev\u0161\u00edm z kvalitn\u00edch b\u00edlkovin, dostatku ovoce a zeleniny a kvalitn\u00edch tuk\u016f, zejm\u00e9na rostlinn\u00fdch a ryb\u00edch. Probl\u00e9mem je kuchy\u0148sk\u00e1 \u00faprava potravin, kter\u00e1 m\u016f\u017ee i ze zdrav\u00ed prosp\u011b\u0161n\u00e9 prom\u011bnit v nezdrav\u00e9. Je zn\u00e1mo, \u017ee nej\u0161kodliv\u011bj\u0161\u00ed je sma\u017een\u00ed, pe\u010den\u00ed a uzen\u00ed, kdy se p\u0159i takov\u00e9 tepeln\u00e9 \u00faprav\u011b vytv\u00e1\u0159ej\u00ed v j\u00eddle karcinogenn\u00ed (rakovinotvorn\u00e9) l\u00e1tky.<\/p>\n<p>\u201ePokud by lid\u00e9 dodr\u017eovali z\u00e1sady zdrav\u00e9 v\u00fd\u017eivy \u2013 tedy jedli kvalitn\u00ed netu\u010dn\u00e9 maso, ryby, dostatek ovoce a zeleniny, vyh\u00fdbali se mou\u010dn\u00fdm a tu\u010dn\u00fdm j\u00eddl\u016fm a potraviny jen va\u0159ili a dusili, byli by nejsp\u00ed\u0161 zdrav\u00ed,\u201c \u0159\u00edk\u00e1 prof. Sva\u010dina.<\/p>\n<p>\u010cesk\u00e1 kuchyn\u011b obsahuje obecn\u011b relativn\u011b nezdrav\u00e1 tu\u010dn\u00e1 j\u00eddla s velk\u00fdm obsahem \u017eivo\u010di\u0161n\u00e9ho tuku. Podobn\u011b jsou na tom i u soused\u016f v Rakousku a Bavorsku. K nespr\u00e1vn\u00e9 skladb\u011b stravy p\u0159isp\u00edv\u00e1 tak\u00e9 pom\u011brn\u011b velk\u00fd pod\u00edl sma\u017een\u00fdch j\u00eddel a uzenin \u2013 ke zdrav\u00ed nej\u0161kodliv\u011bj\u0161\u00edm j\u00eddl\u016fm pat\u0159\u00ed hranolky a uzeniny. Podle prof. Sva\u010diny tyto poznatky \u0159ada lid\u00ed sice dnes u\u017e m\u00e1, ale nedok\u00e1\u017ee svoje stravovac\u00ed n\u00e1vyky zm\u011bnit a z j\u00eddeln\u00ed\u010dku nezdrav\u00e9, ale chutnaj\u00edc\u00ed j\u00eddlo, vy\u0159adit.<\/p>\n<p>Nedostatek v\u016fle zm\u011bnit nejen stravovac\u00ed n\u00e1vyky, ale cel\u00fd \u017eivotn\u00ed styl, je podle d\u011bkana 1. LF UK prof. MUDr. Aleksiho \u0160eda, DrSc., obecn\u011bj\u0161\u00edm probl\u00e9mem. \u201ePacienti o\u010dek\u00e1vaj\u00ed od l\u00e9ka\u0159\u016f \u00fa\u010dinnou l\u00e9\u010dbu, ale ne v\u017edy jsou ochotni podstupovat \u201anepohodl\u00ed\u2018 re\u017eimov\u00fdch opat\u0159en\u00ed a prevence. A p\u0159itom pr\u00e1v\u011b nap\u0159\u00edklad \u201adieta v\u0161edn\u00edho dne\u2018 je z\u00e1sadn\u00ed investic\u00ed pro na\u0161e zdrav\u00ed,\u201c \u0159\u00edk\u00e1 prof. \u0160edo.<\/p>\n<p>Pokud jde o dodr\u017eov\u00e1n\u00ed r\u016fzn\u00fdch diet (vegetari\u00e1nstv\u00ed, veganstv\u00ed, paleodieta, krabi\u010dkov\u00e1 dieta, syrov\u00e1 strava atd.), nab\u00e1d\u00e1 prof. Sva\u010dina k opatrnosti. \u0158ada z nich m\u00e1 podle n\u011bj sice zdrav\u00e9 j\u00e1dro, ale p\u0159i jejich dlouhodob\u00e9m dr\u017een\u00ed hroz\u00ed organismu, \u017ee se mu nedostanou pot\u0159ebn\u00e9 \u017eiviny. Nap\u0159. vegetari\u00e1nsk\u00e1 (bezmas\u00e1) strava je pro dosp\u011bl\u00e9ho celkem zdrav\u00e1, pokud se \u017eivo\u010di\u0161n\u00e9 b\u00edlkoviny nahrazuj\u00ed dostate\u010dn\u011b rostlinn\u00fdmi, ur\u010dit\u011b ale nen\u00ed vhodn\u00e1 pro d\u011bti. Ty pot\u0159ebuj\u00ed pro sv\u016fj v\u00fdvoj i b\u00edlkoviny \u017eivo\u010di\u0161n\u00e9.<\/p>\n<p>V\u00fd\u017eivov\u00ed poradci<\/p>\n<p>Lid\u00e9 dnes st\u00e1le \u010dast\u011bji vyhled\u00e1vaj\u00ed v\u00fd\u017eivov\u00e9 poradce, a to zejm\u00e9na tehdy, kdy\u017e cht\u011bj\u00ed redukovat hmotnost. V\u00fd\u017eivov\u00e9 poradenstv\u00ed m\u016f\u017ee v sou\u010dasnosti podle platn\u00e9 legislativy \u010cR poskytovat kdokoliv, kdo spl\u0148uje v\u0161eobecn\u00e9 podm\u00ednky k z\u00edsk\u00e1n\u00ed \u017eivnostensk\u00e9ho opr\u00e1vn\u011bn\u00ed. Nen\u00ed tedy zdravotn\u00ed slu\u017ebou a nepat\u0159\u00ed mezi regulovan\u00e1 nel\u00e9ka\u0159sk\u00e1 zdravotnick\u00e1 povol\u00e1n\u00ed.<\/p>\n<p>Mezi v\u00fd\u017eivov\u00fdmi poradci tak existuj\u00ed velk\u00e9 rozd\u00edly. Obvykle b\u00fdvaj\u00ed absolventy r\u016fzn\u00fdch kurz\u016f, proto\u017ee jim certifik\u00e1t o absolvov\u00e1n\u00ed kurzu slou\u017e\u00ed p\u0159edev\u0161\u00edm k prok\u00e1z\u00e1n\u00ed odbornosti sv\u00fdm klient\u016fm. V tom lep\u0161\u00edm p\u0159\u00edpad\u011b absolvuj\u00ed akreditovan\u00fd rekvalifika\u010dn\u00ed kurz M\u0160MT \u010cR (dotace 120 hodin).<\/p>\n<p>V\u011bt\u0161inou ov\u0161em postr\u00e1daj\u00ed jakoukoliv skute\u010dnou praxi, resp. ov\u011b\u0159en\u00ed teoretick\u00fdch znalost\u00ed v praxi. K v\u00fdpo\u010dt\u016fm energetick\u00e9ho p\u0159\u00edjmu a v\u00fddeje \u010dasto pou\u017e\u00edvaj\u00ed velmi jednoduch\u00e9 postupy, v\u00fdsledky tak neodpov\u00eddaj\u00ed skute\u010dnosti. Pokud na z\u00e1klad\u011b rozhovoru a neinvazivn\u00ed diagnostiky (nap\u0159. InBody, Bodystat) nab\u00edzej\u00ed i potravinov\u00e9 dopl\u0148ky, jsou s velkou pravd\u011bpodobnost\u00ed jejich distributory. Av\u0161ak p\u0159esto je d\u016fv\u011bra ve v\u00fd\u017eivov\u00e9 poradce pom\u011brn\u011b velk\u00e1 a lid\u00e9 je pova\u017euj\u00ed za odborn\u00edky.<\/p>\n<p>Ze z\u00e1v\u011br\u016f bakal\u00e1\u0159sk\u00e9 pr\u00e1ce absolventky oboru nutri\u010dn\u00ed terapie na 1. LF UK Mgr. Martiny Kollerov\u00e9 z \u010dervence 2015, kter\u00e1 se zab\u00fdvala zkoum\u00e1n\u00edm kvality slu\u017eeb v\u00fd\u017eivov\u00fdch poradc\u016f, vyplynulo, \u017ee se mnoho v\u00fd\u017eivov\u00fdch poradc\u016f dopou\u0161t\u00ed r\u016fzn\u00fdch pochyben\u00ed.<\/p>\n<p>Mezi jin\u00fdmi to bylo nerealistick\u00e9 nastaven\u00ed c\u00edl\u016f hubnut\u00ed u \u010dlov\u011bka s t\u011b\u017ekou obezitou, doporu\u010den\u00ed v\u011bt\u0161\u00ed konzumace tuk\u016f na \u00fakor sacharid\u016f adresovan\u00e9 klientovi s t\u011b\u017ekou obezitou, za\u0159azen\u00ed kokosov\u00e9ho tuku a s\u00e1dla mezi zdrav\u00e9 tuky, doporu\u010den\u00ed detoxika\u010dn\u00ed k\u016fry s velmi n\u00edzk\u00fdm energetick\u00fdm p\u0159\u00edjmem na dobu a\u017e t\u0159i m\u011bs\u00edce, p\u0159\u00edslib hubnut\u00ed p\u0159i u\u017e\u00edv\u00e1n\u00ed dopl\u0148ku stravy bez \u00fapravy stravov\u00e1n\u00ed a pohybov\u00e9ho re\u017eimu.<\/p>\n<p>Psychologick\u00fdm a psychosoci\u00e1ln\u00edm probl\u00e9m\u016fm se v\u00fd\u017eivov\u00ed poradci nev\u011bnuj\u00ed, stejn\u011b tak opom\u00edjej\u00ed ve sv\u00fdch doporu\u010den\u00edch tuto d\u016fle\u017eitou sou\u010d\u00e1st l\u00e9\u010dby a prevence nadv\u00e1hy a obezity.<\/p>\n<p>Nutri\u010dn\u00ed terapeuti<\/p>\n<p>Na rozd\u00edl od v\u00fd\u017eivov\u00fdch poradc\u016f pat\u0159\u00ed nutri\u010dn\u00ed terapeuti k regulovan\u00fdm zdravotnick\u00fdm nel\u00e9ka\u0159sk\u00fdm povol\u00e1n\u00edm. Za v\u00fdkon povol\u00e1n\u00ed nutri\u010dn\u00edho terapeuta se pova\u017euje \u010dinnost v r\u00e1mci preventivn\u00ed p\u00e9\u010de v klinick\u00e9 v\u00fd\u017eiv\u011b a specifick\u00e9 o\u0161et\u0159ovatelsk\u00e9 p\u00e9\u010de zam\u011b\u0159en\u00e9 na uspokojov\u00e1n\u00ed nutri\u010dn\u00edch pot\u0159eb. Ve spolupr\u00e1ci s l\u00e9ka\u0159em se pod\u00edl\u00ed na l\u00e9\u010debn\u00e9 a diagnostick\u00e9 p\u00e9\u010di v oboru klinick\u00e1 v\u00fd\u017eiva. Mus\u00ed absolvovat akreditovan\u00fd studijn\u00ed program (V\u0160) nebo akreditovan\u00fd vzd\u011bl\u00e1vac\u00ed program (S\u0160).<\/p>\n<p>\u00daloha nutri\u010dn\u00edch terapeut\u016f spo\u010d\u00edv\u00e1 jednak v preventivn\u00ed p\u00e9\u010di o zdrav\u00e9 klienty, ale i v p\u00e9\u010di o v\u00fd\u017eivu nemocn\u00fdch. Staraj\u00ed se nap\u0159\u00edklad o v\u00fd\u017eivu pacient\u016f trp\u00edc\u00edch obezitou, cukrovkou, hypertenz\u00ed, nemocemi tr\u00e1vic\u00edho traktu, onkologick\u00fdmi onemocn\u011bn\u00edmi. Nutri\u010dn\u00ed terapeut je tak\u00e9 odborn\u00edkem v p\u0159edopera\u010dn\u00ed a poopera\u010dn\u00ed v\u00fd\u017eiv\u011b \u010di ve v\u00fd\u017eiv\u011b t\u011bhotn\u00fdch a koj\u00edc\u00edch \u017een.<\/p>\n<p>Nap\u0159\u00edklad v USA je povol\u00e1n\u00ed nutri\u010dn\u00edch terapeut\u016f zna\u010dn\u011b roz\u0161\u00ed\u0159eno a tak\u00e9 dob\u0159e finan\u010dn\u011b ohodnoceno. Nutri\u010dn\u00ed terapeuti maj\u00ed sv\u00e9 odborn\u00e9 spole\u010dnosti, vzd\u011bl\u00e1vaj\u00ed se a publikuj\u00ed. Ve srovn\u00e1n\u00ed s t\u00edm zat\u00edm v \u010cesku nen\u00ed tento obor zcela docen\u011bn.<\/p>\n<p>Studium oboru nutri\u010dn\u00ed terapeut a nutri\u010dn\u00ed specialista na 1. LF UK<\/p>\n<p>Na 1. l\u00e9ka\u0159sk\u00e9 fakult\u011b UK je mo\u017en\u00e9 studovat v bakal\u00e1\u0159sk\u00e9m (nutri\u010dn\u00ed terapeut) a navazuj\u00edc\u00edm magistersk\u00e9m (nutri\u010dn\u00ed specialista). Studenti se pod\u00edlej\u00ed i na r\u016fzn\u00fdch epidemiologick\u00fdch v\u00fdzkumech a studi\u00edch. Na 1. LF UK se prov\u00e1d\u011bly nebo je\u0161t\u011b prov\u00e1d\u011bj\u00ed nap\u0159. tyto v\u00fdzkumn\u00e9 projekty: Bariatrick\u00e1 chirurgie a jej\u00ed nutri\u010dn\u00ed aspekty, Dom\u00e1c\u00ed parenter\u00e1ln\u00ed a enter\u00e1ln\u00ed v\u00fd\u017eiva, V\u00fd\u017eiva v intenzivn\u00ed p\u00e9\u010di, Interakce pohybu a v\u00fd\u017eivy, Hormony svalov\u00e9 a tukov\u00e9 tk\u00e1n\u011b a v\u00fd\u017eiva, Z\u00e1n\u011bt vznikaj\u00edc\u00ed po j\u00eddle, V\u00fd\u017eiva a exprese gen\u016f, Organick\u00e9 polutanty ve strav\u011b, Speci\u00e1ln\u00ed diety v l\u00e9\u010db\u011b diabetu a obezity.<\/p>\n<p>S v\u00fdukou prvn\u00edch nutri\u010dn\u00edch terapeut\u016f se na 1. LF UK za\u010dalo p\u0159ed v\u00edce ne\u017e deseti lety. Absolventi maj\u00ed mnoho z\u00e1kladn\u00edch odborn\u00fdch medic\u00ednsk\u00fdch znalost\u00ed se zam\u011b\u0159en\u00edm na klinickou v\u00fd\u017eivu. V sou\u010dasn\u00e9 dob\u011b vych\u00e1z\u00ed z 1. LF UK ro\u010dn\u011b zhruba 50 t\u011bchto odborn\u00edk\u016f. Uplat\u0148uj\u00ed se v nemocnic\u00edch (jen ve V\u0161eobecn\u00e9 fakultn\u00ed nemocnici jich pracuje p\u0159ibli\u017en\u011b 50) i v soukrom\u00fdch ambulanc\u00edch.<\/p>\n<p>\u201eJejich spolupr\u00e1ce s l\u00e9ka\u0159i je v\u00fdhodn\u00e1 pro v\u0161echny strany. Pokud l\u00e9ka\u0159 o\u0161et\u0159uje nap\u0159\u00edklad diabetika, m\u016f\u017ee pacienta v dob\u011b mezi pravideln\u00fdmi kontrolami poslat k n\u00e1v\u0161t\u011bv\u011b nutri\u010dn\u00edho terapeuta. Ten s klientem probere jeho j\u00eddeln\u00ed\u010dek, \u017eivotn\u00ed styl a zvyklosti a d\u00e1 mu fundovan\u00e1 doporu\u010den\u00ed.
