M\u00edcha se na prvn\u00ed pohled zd\u00e1 b\u00fdt v po\u0159\u00e1dku, ve skute\u010dnosti je v\u0161ak zhmo\u017ed\u011bn\u00e1, otekl\u00e1, po\u0161kozen\u00e1 tak, \u017ee nervov\u00e1 spojen\u00ed v n\u00ed u\u017e nemohou p\u0159en\u00e1\u0161et sign\u00e1ly z mozku k centr\u016fm pohybu. Pacient se pravd\u011bpodobn\u011b nebude u\u017e nikdy norm\u00e1ln\u011b h\u00fdbat a Hej\u010dl c\u00edt\u00ed v podobn\u00fdch p\u0159\u00edpadech bezmoc: a\u010dkoli je neurochirurg, soust\u0159edit se m\u016f\u017ee jen na n\u00e1pravu a stabilizaci p\u00e1te\u0159e. &#8222;S m\u00edchou ned\u011bl\u00e1me nic,&#8220; \u0159\u00edk\u00e1 nespokojen\u011b a p\u0159ipom\u00edn\u00e1, \u017ee jen v \u010cesku si m\u00edchu poran\u00ed zhruba 200\u2013300 lid\u00ed ka\u017ed\u00fd rok. Celosv\u011btov\u011b je to a\u017e p\u016fl milionu p\u0159\u00edpad\u016f ro\u010dn\u011b.<br \/>\nOpravit ji, obnovit v n\u00ed nervov\u00e1 spojen\u00ed l\u00e9ka\u0159sk\u00e1 v\u011bda zat\u00edm neum\u00ed a s v\u00fdjimkou n\u011bkolika vz\u00e1cn\u00fdch klinick\u00fdch studi\u00ed se o to u lid\u00ed ani nepokou\u0161\u00ed. O to fantasti\u010dt\u011bji vypad\u00e1 v\u00fdzkum uskute\u010dn\u011bn\u00fd ve \u0160v\u00fdcarsku a publikovan\u00fd loni na podzim v \u010dasopise Nature. Krom\u011b jist\u00e9 nad\u011bje pro paralyzovan\u00e9 p\u0159in\u00e1\u0161\u00ed tak\u00e9 \u00fadiv, kam dosp\u011blo propojov\u00e1n\u00ed \u017eiv\u00e9ho organismu a stroje.<\/p>\n<p>Stiskem tla\u010d\u00edtka Ovl\u00e1d\u00e1n\u00ed pohybu bez p\u0159\u00edm\u00e9ho nervov\u00e9ho spojen\u00ed, pouhou silou my\u0161lenky, je \u010dast\u00fdm n\u00e1m\u011btem sci-filiteratury. Figuruje t\u0159eba ve slavn\u00e9 novele Arthura Clarka Setk\u00e1n\u00ed s med\u00fazou z roku 1971 \u010di ve star\u0161\u00edm p\u0159\u00edb\u011bhu Poula Andersona \u0158\u00edkejte mi Joe, j\u00edm\u017e se nejsp\u00ed\u0161 inspirovali tv\u016frci filmu Avatar. N\u00e1m\u011bt je v\u017edy podobn\u00fd: paralyzovan\u00fd, ch\u016fze neschopn\u00fd \u010dlov\u011bk ovl\u00e1d\u00e1 sv\u00fdm mozkem stroj, p\u0159\u00edpadn\u011b um\u011ble vytvo\u0159en\u00e9 t\u011blo.<br \/>\nK napln\u011bn\u00ed t\u00e9to vize se p\u0159ibl\u00ed\u017eili nap\u0159\u00edklad v\u011bdci z Pittsbursk\u00e9 univerzity v USA, kte\u0159\u00ed v roce 2012 pomoc\u00ed magnetick\u00e9 rezonance zmapovali, kter\u00e9 oblasti mozku \u0159\u00edd\u00ed pohyb ruky, a pot\u00e9 do nich zavedli mikroskopick\u00e9 elektrody schopn\u00e9 ode\u010d\u00edst nervov\u00e9 sign\u00e1ly a p\u0159edat je po\u010d\u00edta\u010di. Dvaapades\u00e1tilet\u00e9 pacientce jm\u00e9nem Jan Scheuermann ochrnut\u00e9 od krku dol\u016f tak prop\u016fj\u010dili schopnost ovl\u00e1dat robotickou ruku: h\u00fdbala v duchu skute\u010dnou pa\u017e\u00ed a elektrody v jej\u00ed mozkov\u00e9 k\u016f\u0159e p\u0159en\u00e1\u0161ely pohyb na mechanickou kon\u010detinu um\u00edst\u011bnou na podstavci vedle jej\u00edho voz\u00edku.<br \/>\nPom\u011brn\u011b p\u0159irozen\u011b, v podstat\u011b b\u011b\u017enou rychlost\u00ed tak dok\u00e1zala uchopit p\u0159edm\u011bt a obr\u00e1tit jej vzh\u016fru nohama nebo si vlo\u017eit do \u00fast \u010dokol\u00e1du. &#8222;H\u00fdbu v\u011bcmi. Deset let jsem ni\u010d\u00edm nepohnula,&#8220; l\u00ed\u010dila tehdy usm\u011bvav\u00e1 \u017eena sv\u00e9 pocity m\u00e9di\u00edm.<br \/>\nExperiment ov\u0161em prob\u00edhal v laborato\u0159i, za asistence t\u00fdmu technik\u016f a p\u016fsobil i trochu d\u011bsiv\u011b \u2013 nejen kv\u016fli mechanick\u00e9 ruce, ale tak\u00e9 proto, \u017ee pacientce vych\u00e1zely z temene dva svazky kabel\u016f. U\u017e tehdy se mluvilo o tom, \u017ee by to m\u011blo j\u00edt elegantn\u011bji, bezdr\u00e1tov\u011b, a \u017ee by p\u0159\u00edjemcem sign\u00e1l\u016f nemusel b\u00fdt robot, ale s\u00e1m posti\u017een\u00fd \u010dlov\u011bk. A opatrn\u011b se za\u010dala zmi\u0148ovat mo\u017enost pomoci takto lidem po \u00farazu m\u00edchy.<br \/>\nK n\u011b\u010demu takov\u00e9mu je zat\u00edm velmi daleko, lo\u0148sk\u00fd rok byl v\u0161ak d\u00edky zm\u00edn\u011bn\u00e9mu v\u00fdzkumu pr\u016flomov\u00fd. \u010cl\u00e1nek doprov\u00e1z\u00ed pozoruhodn\u00e9 video, na n\u011bm\u017e kr\u00e1\u010d\u00ed v laborato\u0159i na pohybliv\u00e9m chodn\u00edku makak; t\u00e9m\u011b\u0159 neznateln\u011b kulh\u00e1, jinak se zd\u00e1 b\u00fdt v po\u0159\u00e1dku. N\u00e1hle prav\u00e1 zadn\u00ed noha znehybn\u00ed, opice se za\u010dne pohybovat s nejv\u011bt\u0161\u00edmi obt\u00ed\u017eemi, po t\u0159ech. Sta\u010dilo stisknout vyp\u00edna\u010d a vy\u0159adit z provozu za\u0159\u00edzen\u00ed, kter\u00e9 bezdr\u00e1tov\u011b p\u0159en\u00e1\u0161\u00ed sign\u00e1ly z mozku zv\u00ed\u0159ete do implant\u00e1t\u016f v bedern\u00ed oblasti. Makak s po\u0161kozenou m\u00edchou tak ihned naplno poc\u00edt\u00ed n\u00e1sledky zran\u011bn\u00ed, kv\u016fli n\u011bmu\u017e nem\u016f\u017ee h\u00fdbat pravou zadn\u00ed nohou.<br \/>\n&#8222;Byl jsem velmi skeptick\u00fd. Nev\u011b\u0159il jsem, \u017ee to bude fungovat, rozhodn\u011b ne tak dob\u0159e,&#8220; \u0159\u00edk\u00e1 do telefonu bioin\u017een\u00fdr Marco Capogrosso ze \u0160v\u00fdcarsk\u00e9ho feder\u00e1ln\u00edho technologick\u00e9ho institutu (EPFL). \u0160\u00e9f t\u00fdmu \u00fasp\u011bchem mysl\u00ed hlavn\u011b to, \u017ee po op\u011btovn\u00e9m &#8222;stisknut\u00ed tla\u010d\u00edtka&#8220; a zapnut\u00ed bezdr\u00e1tov\u00e9ho mostu mezi mozkem a bedern\u00ed \u010d\u00e1st\u00ed m\u00edchy m\u016f\u017ee zv\u00ed\u0159e okam\u017eit\u011b pokra\u010dovat norm\u00e1ln\u00ed ch\u016fz\u00ed.<\/p>\n<p>V\u00edc ne\u017e sada kabel\u016f &#8222;Metoda m\u016f\u017ee fungovat i v p\u0159\u00edpadech, kdy byla p\u0159eru\u0161ena \u00fapln\u011b v\u0161echna nervov\u00e1 spojen\u00ed v m\u00ed\u0161e,&#8220; komentuje na str\u00e1nk\u00e1ch Nature v\u00fdzkum optimisticky neurov\u011bdec Andrew Jackson z Newcastlesk\u00e9 univerzity.<br \/>\nTechnologii bezdr\u00e1tov\u00e9ho mostu piloval \u0161v\u00fdcarsk\u00fd t\u00fdm mnoho let, jen\u017ee na potkanech. Prim\u00e1ti, t\u0159eba pr\u00e1v\u011b opice a lid\u00e9, v\u0161ak \u0159\u00edd\u00ed pohyb dost odli\u0161n\u011b \u2013 a pr\u00e1v\u011b odtud pramenila Capogrossova skepse. &#8222;M\u00e1me v\u00edce mozkov\u00fdch struktur, takov\u00fdch, jak\u00e9 u potkan\u016f nejsou,&#8220; \u0159\u00edk\u00e1 p\u016fvodem italsk\u00fd v\u011bdec. P\u0159itom \u0161lo pr\u00e1v\u011b o to, jak z motorick\u00e9 k\u016fry mozku ode\u010d\u00edst \u00famysl ud\u011blat krok, pak sign\u00e1ly dek\u00f3dovat a p\u0159en\u00e9st do bedern\u00ed oblasti m\u00edchy.<br \/>\nPro\u010d pr\u00e1v\u011b sem? &#8222;Lid\u00e9 se obvykle domn\u00edvaj\u00ed, \u017ee m\u00edcha je pouhou sadou kabel\u016f, kter\u00e9 vedou z mozku ke sval\u016fm. Ne\u017e jsem zah\u00e1jil tenhle v\u00fdzkum, myslel jsem si to tak\u00e9. To je ale omyl: nervov\u00e1 s\u00ed\u0165, kter\u00e1 \u0159\u00edd\u00ed pohyb kon\u010detin, je pr\u00e1v\u011b v m\u00ed\u0161e,&#8220; vysv\u011btluje Capogrosso. Zran\u011bn\u00ed ji odd\u011bl\u00ed od mozku, tak\u017ee nedost\u00e1v\u00e1 \u017e\u00e1dn\u00e9 sign\u00e1ly. &#8222;Ty bu\u0148ky v\u0161ak nejsou mrtv\u00e9. Jen s nimi nikdo nekomunikuje.&#8220;<br \/>\nObnovit komunikaci se v\u011bdci pokou\u0161ej\u00ed po desetilet\u00ed nejr\u016fzn\u011bj\u0161\u00edmi metodami, z nich\u017e v\u011bt\u0161ina nem\u00e1 s elektronikou ani softwarem nic spole\u010dn\u00e9ho. \u010cist\u011b biologick\u00fd v\u00fdzkum, jak lidem s po\u0161kozenou m\u00edchou pomoci, z\u00edskal velk\u00fd impulz v devades\u00e1t\u00fdch letech minul\u00e9ho stolet\u00ed, kdy se uk\u00e1zalo, \u017ee v t\u011ble existuj\u00ed kmenov\u00e9 bu\u0148ky \u2013 schopn\u00e9 zm\u011bnit se na bu\u0148ky po\u0161kozen\u00e9ho org\u00e1nu a t\u00edm jej opravit. &#8222;Byla to nov\u00e1 nad\u011bje,&#8220; vzpom\u00edn\u00e1 Ale\u0161 Hej\u010dl. &#8222;Vypadalo to, \u017ee l\u00e9ka\u0159i je p\u00edchnou do m\u00edchy a ony budou tvo\u0159it nov\u00e9 neurony. Postupn\u011b se ale uk\u00e1zalo, \u017ee to nen\u00ed tak jednoduch\u00e9,&#8220; vysv\u011btluje neurochirurg nemocnice v \u00dast\u00ed nad Labem, kter\u00fd v pra\u017esk\u00e9m \u00dastavu experiment\u00e1ln\u00ed medic\u00edny AV \u010cR zkoum\u00e1 jinou metodu \u2013 biomateri\u00e1ly, takzvan\u00e9 hydrogely, kter\u00e9 lze vlo\u017eit do zran\u011bn\u00e9 m\u00edchy, a proto\u017ee jsou por\u00e9zn\u00ed, vytvo\u0159\u00ed syst\u00e9m kan\u00e1lk\u016f, jimi\u017e mohou p\u0159eru\u0161en\u00e9 nervy op\u011btovn\u011b pror\u016fstat a propojovat se.<br \/>\nBiologick\u00e9 metody dok\u00e1\u017eou m\u00edrn\u011b zlep\u0161it stav potkan\u016f, co\u017e ale neznamen\u00e1, \u017ee budou fungovat tak\u00e9 u lid\u00ed. Jestli pacient\u016fm pom\u00e1haj\u00ed, nev\u00edme \u2013 nap\u0159\u00edklad u kmenov\u00fdch bun\u011bk podle Ale\u0161e Hej\u010dla klinick\u00e9 studie zat\u00edm ov\u011b\u0159ovaly jen bezpe\u010dnost metody, hlavn\u011b to, zda implantovan\u00e9 bu\u0148ky nevytv\u00e1\u0159ej\u00ed zhoubn\u00e9 n\u00e1dory. V tomto ohledu m\u016f\u017eeme b\u00fdt z\u0159ejm\u011b klidn\u00ed, prvn\u00ed pacienti jsou sledov\u00e1ni u\u017e deset let a rakovina se neobjevila. Z\u00e1zra\u010dn\u00e9 \u00fa\u010dinky v\u0161ak l\u00e9ka\u0159i nepozoruj\u00ed a p\u0159\u00edpadn\u00e9 m\u00edrn\u00e9 zlep\u0161en\u00ed z\u016fst\u00e1v\u00e1 poh\u0159beno v chaosu dat, kdy do studi\u00ed byli nab\u00edr\u00e1ni pacienti s velmi odli\u0161n\u00fdm druhem poran\u011bn\u00ed. V t\u00e9to nejasn\u00e9 situaci p\u0159in\u00e1\u0161\u00ed experiment \u0161v\u00fdcarsk\u00e9ho t\u00fdmu n\u011bco \u00fapln\u011b nov\u00e9ho.<\/p>\n<p>P\u0159\u00edm\u011bj\u0161\u00ed cesta &#8222;Vn\u00edm\u00e1m jej jako p\u0159\u00edm\u011bj\u0161\u00ed zp\u016fsob. P\u0159ek\u00e1\u017eek nen\u00ed m\u00e1lo, ale jestli je mo\u017en\u00e9 usp\u011bt, m\u00e1 tahle cesta mo\u017en\u00e1 v\u011bt\u0161\u00ed \u0161anci,&#8220; p\u0159ipou\u0161t\u00ed Ale\u0161 Hej\u010dl, podle n\u011bho\u017e si lze p\u0159edstavit, \u017ee l\u00e9ka\u0159i v budoucnu zvol\u00ed kombinaci obou metod. M\u016f\u017ee se nap\u0159\u00edklad uk\u00e1zat, \u017ee m\u00edcha regeneruje nejl\u00e9pe tehdy, dostane-li se j\u00ed biologick\u00e9ho l\u00e9ku kombinovan\u00e9ho s biomateri\u00e1ly \u2013 a z\u00e1rove\u0148 pacient bude moci pohybovat nohama d\u00edky bezdr\u00e1tov\u00e9mu mostu mezi mozkem a centry pohybu v m\u00ed\u0161e.<br \/>\nCapogross\u016fv experiment je ostatn\u011b i vyvrcholen\u00edm dlouholet\u00fdch snah, jak tato centra, kter\u00e1 \u00faraz odd\u011blil od mozku, zven\u010d\u00ed probudit a p\u0159im\u011bt k \u010dinnosti. U\u017e n\u011bkolik des\u00edtek rok\u016f se je v\u011bdci sna\u017e\u00ed stimulovat pomoc\u00ed elektrod implantovan\u00fdch do p\u00e1te\u0159e nebo um\u00edst\u011bn\u00fdch na k\u016f\u017ei. Proud, kter\u00fd elektrody do organismu pou\u0161t\u011bj\u00ed, je v\u0161ak v tomto p\u0159\u00edpad\u011b &#8222;hloup\u00fd&#8220;, neobsahuje \u017e\u00e1dn\u00e9 instrukce. Sice se n\u011bkter\u00fdm paraplegik\u016fm poda\u0159ilo vr\u00e1tit schopnost do jist\u00e9 m\u00edry h\u00fdbat nohama, k ch\u016fzi je v\u0161ak odtud velmi daleko.<br \/>\nPro ni je toti\u017e d\u016fle\u017eit\u00e9 zapojen\u00ed mozku. A prvn\u00edm, kdo jej zprost\u0159edkoval, byl pr\u00e1v\u011b t\u00fdm z EPFL: elektrody v mozkov\u00e9 k\u016f\u0159e makaka zaznamenaly p\u0159\u00edslu\u0161n\u00fd sign\u00e1l, p\u0159es wi-fispojen\u00ed jej vyslaly do implant\u00e1tu v bedern\u00ed p\u00e1te\u0159i a ten podr\u00e1\u017edil zm\u00edn\u011bn\u00e9 bu\u0148ky v m\u00ed\u0161e tak, aby p\u0159ik\u00e1zaly sval\u016fm ud\u011blat krok. V\u011bdci d\u00edky tomu mohli nejen &#8222;zapnout&#8220; a &#8222;vypnout&#8220; opi\u010d\u00ed ch\u016fzi, dok\u00e1zali tak\u00e9 zlep\u0161it pohyb pomoc\u00ed p\u0159esn\u011bj\u0161\u00ed modulace p\u0159en\u00e1\u0161en\u00fdch mozkov\u00fdch sign\u00e1l\u016f. Dokonce byli \u00famysl zv\u00ed\u0159ete p\u0159eru\u0161it odpo\u010dinek a pokra\u010dovat v ch\u016fzi \u2013 zaznamenat jej o zlomek vte\u0159iny d\u0159\u00edv, ne\u017e to opice skute\u010dn\u011b ud\u011blala.<\/p>\n<p>Rad\u011bji v \u010c\u00edn\u011b Krom\u011b \u00fadivu nad mo\u017enostmi techniky tu ov\u0161em lze poc\u00edtit i jist\u00e9 zd\u011b\u0161en\u00ed, jak snadno m\u016f\u017ee \u017eiv\u00fd tvor za\u010d\u00edt fungovat &#8222;na d\u00e1lkov\u00e9 ovl\u00e1d\u00e1n\u00ed&#8220;. Pominout nelze ani skute\u010dnost, \u017ee \u010d\u00e1ste\u010dn\u00e9 poran\u011bn\u00ed m\u00edchy zp\u016fsobili v\u011bdci opic\u00edm \u00famysln\u011b ve chv\u00edli, kdy m\u011bli aparaturu vylad\u011bnou a kdy p\u0159ed\u00e1v\u00e1n\u00ed sign\u00e1l\u016f mezi mozkov\u00fdm a bedern\u00edm implant\u00e1tem dob\u0159e rozum\u011bli. Podle Marca Capogrossa to jinak ne\u0161lo, potkani se od prim\u00e1t\u016f p\u0159\u00edli\u0161 li\u0161\u00ed, aby bylo mo\u017en\u00e9 experimentovat pouze s nimi a pak p\u0159ej\u00edt rovnou na lidsk\u00e9ho pacienta. Ob\u011b poran\u011bn\u00e9 opice se nav\u00edc po odstran\u011bn\u00ed implant\u00e1tu za n\u011bkolik m\u011bs\u00edc\u016f uzdrav\u00ed \u2013 jejich ch\u016fze sice nebude \u00fapln\u011b norm\u00e1ln\u00ed, budou se v\u0161ak moci pohybovat samostatn\u011b a bez omezen\u00ed. Organismus se nau\u010d\u00ed ovl\u00e1dat kon\u010detinu pomoc\u00ed t\u011bch spojen\u00ed mezi mozkem a m\u00edchou, kter\u00e1 p\u0159eru\u0161ena nebyla.<br \/>\nUvnit\u0159 v\u011bdeck\u00e9 komunity se nicm\u00e9n\u011b diskutuje o tom, zda nen\u00ed mo\u017en\u00e9 se bez prim\u00e1t\u016f p\u0159ece jen obej\u00edt. D\u016fvodem nejsou ani tak ohledy na pr\u00e1va zv\u00ed\u0159at \u2013 z \u010dist\u011b z biologick\u00e9ho pohledu nen\u00ed d\u016fvod si myslet, \u017ee potkan trp\u00ed m\u00e9n\u011b ne\u017e opice \u2013, ale sp\u00ed\u0161e s\u00edl\u00edc\u00ed bari\u00e9ry, kter\u00e9 klade pokus\u016fm s prim\u00e1ty do cesty legislativa, ve\u0159ejn\u00e9 m\u00edn\u011bn\u00ed nebo i postoj v\u011bt\u0161iny leteck\u00fdch spole\u010dnost\u00ed, kter\u00e9 odm\u00edtaj\u00ed prim\u00e1ty pro v\u00fdzkum p\u0159epravovat. Ostatn\u011b a\u010dkoli \u0161v\u00fdcarsk\u00fd experiment vyhov\u011bl evropsk\u00fdm etick\u00fdm standard\u016fm, uskute\u010dnil se v \u010c\u00edn\u011b, kde nehroz\u00ed protesty ve\u0159ejnosti a p\u0159\u00edstup \u00fa\u0159ad\u016f nen\u00ed tak p\u0159\u00edsn\u00fd.<br \/>\nA\u0165 u\u017e ale budou pokusy na opic\u00edch pokra\u010dovat, nebo se v\u011bdci pod tlakem ve\u0159ejn\u00e9ho m\u00edn\u011bn\u00ed rozhodnou p\u0159ej\u00edt od potkan\u016f rovnou k lidem, jde o b\u011bh na dlouhou tra\u0165. Podle Marca Capogrossa potrv\u00e1 p\u011bt a\u017e deset let, ne\u017e technologie bezdr\u00e1tov\u00e9ho mostu dosp\u011bje natolik, aby mohla b\u00fdt vyzkou\u0161ena u \u010dlov\u011bka. I testy si pak vy\u017e\u00e1daj\u00ed hodn\u011b \u010dasu \u2013 a za\u010dnou, jen pokud se najde farmaceutick\u00e1 firma ochotn\u00e1 je p\u0159evz\u00edt; pro univerzitn\u00ed laborato\u0159 je to p\u0159\u00edli\u0161 velk\u00e9 sousto.
