Na\u0161e t\u011bla p\u0159itom obsahuj\u00ed cenn\u00e9 bu\u0148ky, kter\u00e9 mohou b\u00fdt v budoucnu kl\u00ed\u010dem k l\u00e9\u010db\u011b nebo dokonce z\u00e1chran\u011b \u017eivota n\u00e1s \u010di na\u0161ich bl\u00edzk\u00fdch. V\u00fdzvou je spr\u00e1vn\u00e9 zpracov\u00e1n\u00ed a uchov\u00e1n\u00ed bun\u011bk, aby si zachovaly sv\u00e9 unik\u00e1tn\u00ed vlastnosti. Jak\u00e9 druhy biologick\u00e9ho materi\u00e1lu je mo\u017en\u00e9 zmrazit nebo jinak uchovat pro budouc\u00ed l\u00e9\u010debn\u00e9 \u00fa\u010dely?<\/strong><\/p>\n\n\n\n<p><strong>KOSTN\u00cd D\u0158E\u0147<\/strong><\/p>\n\n\n\n<p>Nejstar\u0161\u00ed a nejv\u00edce prozkouman\u00fd zdroj bun\u011bk je kostn\u00ed d\u0159e\u0148.&nbsp;Obsahuje krvetvorn\u00e9 kmenov\u00e9 bu\u0148ky, kter\u00e9 jsou kl\u00ed\u010dov\u00e9 pro tvorbu krve. Zdrav\u00e9 bu\u0148ky mohou poch\u00e1zet od d\u00e1rce nebo z t\u011bla pacienta p\u0159ed chemoterapi\u00ed nebo radioterapi\u00ed.<\/p>\n\n\n\n<p><strong>SPERMIE<\/strong><\/p>\n\n\n\n<p>Kryokonzervace spermi\u00ed umo\u017e\u0148uje zachovat \u0161ance na po\u010det\u00ed biologick\u00e9ho d\u00edt\u011bte v budoucnu. Tento proces se vyu\u017e\u00edv\u00e1 v\u00edce ne\u017e \u010dty\u0159icet let, zejm\u00e9na pro mu\u017ee podstupuj\u00edc\u00ed l\u00e9\u010dbu rakoviny, s rizikov\u00fdm povol\u00e1n\u00edm nebo p\u0159ed l\u00e9ka\u0159sk\u00fdm z\u00e1krokem ovliv\u0148uj\u00edc\u00edm plodnost. Mnoho mu\u017e\u016f p\u0159istupuje ke zmrazov\u00e1n\u00ed spermi\u00ed i pro d\u00e1rcovsk\u00e9 \u00fa\u010dely. Spermie se skladuj\u00ed v modern\u00edch spermobank\u00e1ch pomoc\u00ed dus\u00edkov\u00fdch par a mohou b\u00fdt pou\u017eity i po v\u00edce ne\u017e deseti letech. Nejjednodu\u0161\u0161\u00ed zp\u016fsob, jak z\u00edskat vzorek pro zmrazen\u00ed, je masturbace. Zmrazen\u00ed kvalitu spermi\u00ed neovliv\u0148uje. Po rozmrazen\u00ed jsou stejn\u011b \u00fa\u010dinn\u00e9 jako \u010derstv\u00e9. Etick\u00e9 ot\u00e1zky zahrnuj\u00ed anonymitu d\u00e1rcovstv\u00ed a pou\u017eit\u00ed spermi\u00ed po smrti d\u00e1rce bez jeho souhlasu.<\/p>\n\n\n\n<p><strong>VAJ\u00cd\u010cKA<\/strong><\/p>\n\n\n\n<p>\u017deny mohou zachovat svou plodnost zmrazen\u00edm vaj\u00ed\u010dek. \u00dasp\u011b\u0161nost tohoto postupu v\u00fdrazn\u011b z\u00e1vis\u00ed na v\u011bku \u017eeny. \u010c\u00edm d\u0159\u00edve se rozhodne, t\u00edm vy\u0161\u0161\u00ed m\u00e1 \u0161anci na \u00fasp\u011bch. Nej\u010dast\u011bji se pro zmrazen\u00ed vaj\u00ed\u010dek rozhodnou \u017eeny p\u0159ed onkologickou l\u00e9\u010dbou, chemoterapi\u00ed nebo radioterapi\u00ed, nebo ty, kter\u00e9 aktu\u00e1ln\u011b nemaj\u00ed partnera a ob\u00e1vaj\u00ed se, \u017ee by mohly m\u00edt v budoucnu pot\u00ed\u017ee s ot\u011bhotn\u011bn\u00edm.<\/p>\n\n\n\n<p><strong>EMBRYA<\/strong><\/p>\n\n\n\n<p>Embrya jsou \u010dasto zmrazov\u00e1na b\u011bhem procesu asistovan\u00e9 reprodukce, jako je IVF (in vitro fertilizace). Mohou b\u00fdt skladov\u00e1na po mnoho let a pou\u017eita k implantaci v budoucnosti. V \u010cR jsou standardn\u011b skladov\u00e1na t\u0159i roky, z\u00e1le\u017e\u00ed v\u0161ak na rodi\u010d\u00edch, jak dlouho je\/bude embryo skladov\u00e1no. Po uplynut\u00ed doby 3 let je na rozhodnut\u00ed rodi\u010d\u016f, zda bude doba prodlou\u017eena. Kvalitu embrya d\u00e9lka zmrazen\u00ed nijak neovliv\u0148uje, za nejrizikov\u011bj\u0161\u00ed se pova\u017euje proces zmra\u017een\u00ed, kter\u00fd m\u016f\u017ee zp\u016fsobit po\u0161kozen\u00ed. Del\u0161\u00ed doba skladov\u00e1n\u00ed v\u0161ak embryo nijak nepo\u0161kozuje a zdrav\u00e9 d\u00edt\u011b se m\u016f\u017ee narodit i z embrya zmra\u017een\u00e9ho des\u00edtky let. Embrya mohou b\u00fdt uchov\u00e1v\u00e1na metodou&nbsp;kryokonzervace. P\u0159i t\u00e9to metod\u011b jsou zmrazena v tekut\u00e9m dus\u00edku p\u0159i teplot\u011b \u2212196 \u00b0C ve speci\u00e1ln\u00edch n\u00e1dob\u00e1ch. D\u0159\u00edve pou\u017e\u00edvanou metodu pomal\u00e9ho mra\u017een\u00ed nahradila&nbsp;vitrifikace, kter\u00e1 minimalizuje riziko po\u0161kozen\u00ed bun\u011bk. Embrya zmrazen\u00e1 touto metodou lze uchov\u00e1vat i n\u011bkolik des\u00edtek let. Kdy\u017e p\u0159ijde ta spr\u00e1vn\u00e1 chv\u00edle, jsou snadno rozmrazena a pou\u017eita. Pravd\u011bpodobnost \u00fasp\u011b\u0161n\u00e9ho uchycen\u00ed je srovnateln\u00e1 s \u010derstv\u00fdmi embryi.<\/p>\n\n\n\n<p><strong>PUPO\u010cN\u00cdKOV\u00c1 KREV A TK\u00c1\u0147 PUPE\u010cN\u00cdKA<\/strong><\/p>\n\n\n\n<p>Pupe\u010dn\u00edkov\u00e1 krev obsahuje r\u016fzn\u00e9 typy bun\u011bk, v sou\u010dasnosti jsou nejv\u00edce pou\u017e\u00edv\u00e1ny krvetvorn\u00e9 kmenov\u00e9 bu\u0148ky. Jej\u00ed odb\u011br a ulo\u017een\u00ed je proto mo\u017en\u00e9 jen jednou \u2013 b\u011bhem porodu. Bezprost\u0159edn\u011b po porodu se krev odebere z pupe\u010dn\u00edkov\u00e9 \u017e\u00edly do odb\u011brov\u00e9ho vaku s antikoagulantem, aby se zabr\u00e1nilo sr\u00e1\u017een\u00ed krve. Odb\u011br se prov\u00e1d\u00ed a\u017e po p\u0159est\u0159i\u017een\u00ed pupe\u010dn\u00ed \u0161\u0148\u016fry, pro matku i d\u00edt\u011b je tedy bezbolestn\u00fd. Pokud se z\u00e1rove\u0148 odeb\u00edr\u00e1 i tk\u00e1\u0148 pupe\u010dn\u00edku, \u010d\u00e1st z n\u011bho se odst\u0159ihne steriln\u00edmi n\u016f\u017ekami a vlo\u017e\u00ed se do steriln\u00ed zkumavky s transportn\u00edm m\u00e9diem. Odebran\u00e1 pupe\u010dn\u00edkov\u00e1 krev a tk\u00e1\u0148 se pot\u00e9 pe\u010dliv\u011b zabal\u00ed a p\u0159enesou do specializovan\u00e9 laborato\u0159e nebo banky pupe\u010dn\u00edkov\u00e9 krve. Je d\u016fle\u017eit\u00e9, aby pupe\u010dn\u00edkov\u00e1 krev i tk\u00e1\u0148 pro\u0161ly steriln\u00edm a kontrolovan\u00fdm procesem zpracov\u00e1n\u00ed a uskladnily se co nejd\u0159\u00edve po odb\u011bru \u2013 zpracov\u00e1ny v laborato\u0159i by m\u011bly b\u00fdt do 72 hodin od narozen\u00ed d\u00edt\u011bte. Po zmrazen\u00ed mohou b\u00fdt skladov\u00e1ny ve speci\u00e1ln\u00edch kontejnerech v par\u00e1ch tekut\u00e9ho dus\u00edku s teplotou -196 C \u00b0 mnoho let, a\u017e stolet\u00ed. Pupe\u010dn\u00edkov\u00e1 krev se usklad\u0148uje v rodinn\u00e9 bance pupe\u010dn\u00edkov\u00e9 krve pro pot\u0159eby d\u00edt\u011bte, kter\u00e9mu byla odebr\u00e1na nebo pro pot\u0159eby sourozence.<\/p>\n\n\n\n<p><strong>PLACENTA<\/strong><\/p>\n\n\n\n<p>Spolu s pupe\u010dn\u00edkovou krv\u00ed \u010di tk\u00e1n\u00ed pupe\u010dn\u00edku se m\u016f\u017ee uchovat i placenta. Tato mo\u017enost nast\u00e1v\u00e1 pouze jednou \u2013 bezprost\u0159edn\u011b po narozen\u00ed d\u00edt\u011bte. Placenta je bohat\u00e1 na r\u016fzn\u00e9 typy bun\u011bk. Odb\u011br placenty nen\u00ed obzvl\u00e1\u0161t\u011b n\u00e1ro\u010dn\u00fd, do steriln\u00ed odb\u011brov\u00e9 soupravy se odebere bezprost\u0159edn\u011b po odb\u011bru pupe\u010dn\u00edkov\u00e9 krve. Nezat\u011b\u017euje matku ani d\u00edt\u011b, je naprosto bezpe\u010dn\u00fd a bezbolestn\u00fd. Oproti zpracov\u00e1n\u00ed pupe\u010dn\u00edkov\u00e9 krve a tk\u00e1n\u011b pupe\u010dn\u00edku, je v\u0161ak zpracov\u00e1n\u00ed placenty p\u0159ed zamra\u017een\u00edm mnohem n\u00e1ro\u010dn\u011bj\u0161\u00ed. Po zpracov\u00e1n\u00ed je rozd\u011blena a uchov\u00e1na ve v\u00edce vac\u00edch a zkumavk\u00e1ch (dle velikosti placenty) p\u0159i teplot\u011b -196\u00b0C. Ob\u011b vrstvy placenty \u2013 vnit\u0159n\u00ed (amnion) i vn\u011bj\u0161\u00ed (chorion) maj\u00ed siln\u00e9 regenera\u010dn\u00ed vlastnosti. V klinick\u00fdch studi\u00edch po cel\u00e9m sv\u011bt\u011b v\u011bdci zji\u0161\u0165uj\u00ed, p\u0159i kter\u00fdch konkr\u00e9tn\u00edch onemocn\u011bn\u00edch m\u00e1 smysl bu\u0148ky placenty vyu\u017e\u00edt. Placenta, stejn\u011b jako pupe\u010dn\u00edkov\u00e1 krev a pupe\u010dn\u00edk, obsahuje mlad\u00e9 bu\u0148ky, kter\u00e9 nejsou zat\u00ed\u017eeny prod\u011blan\u00fdmi onemocn\u011bn\u00edmi, l\u00e9\u010dbou ani procesem st\u00e1rnut\u00ed co se v l\u00e9\u010db\u011b jev\u00ed jako velk\u00fd benefit. V\u00fdhodou je i to, \u017ee jej bu\u0148ky maj\u00ed n\u00edzkou imunogenecitu, to znamen\u00e1, \u017ee je lze pou\u017e\u00edt nejen pro d\u00edt\u011b, kter\u00e9mu tato placenta poskytla v\u0161e pot\u0159ebn\u00e9 v obdob\u00ed nitrod\u011blo\u017en\u00edho v\u00fdvoje, ale tak\u00e9 pro jeho sourozence \u010di rodi\u010de.<\/p>\n\n\n\n<p><strong>KREV A JEJ\u00cd SLO\u017dKY<\/strong><\/p>\n\n\n\n<p>Darov\u00e1n\u00ed krve by nem\u011blo \u017e\u00e1dn\u00fd v\u00fdznam, pokud by se d\u016fkladn\u011b neuskladnila. Krev m\u016f\u017ee b\u00fdt odeb\u00edr\u00e1na spolu se v\u0161emi sv\u00fdmi slo\u017ekami, nebo se tyto slo\u017eky p\u0159i odb\u011bru z\u00edsk\u00e1vaj\u00ed samostatn\u011b. \u010cerven\u00e9 krvinky se skladuj\u00ed p\u0159i teplot\u011b 2 a\u017e 6 \u00b0C maxim\u00e1ln\u011b 42 dn\u00ed a vyu\u017eit\u00ed najdou p\u0159i l\u00e9\u010db\u011b ztr\u00e1t krve p\u0159i krv\u00e1cen\u00ed a t\u011b\u017ek\u00e9 chudokrevnosti. B\u00edl\u00e9 krvinky se mus\u00ed zpracovat do n\u011bkolika hodin od odebr\u00e1n\u00ed a pou\u017e\u00edvaj\u00ed se jen ve velmi v\u00fdjime\u010dn\u00fdch p\u0159\u00edpadech. Krevn\u00ed desti\u010dky vydr\u017e\u00ed teploty v rozmez\u00ed 21 a\u017e 25 \u00b0C a jejich \u017eivotnost je p\u011bt dn\u00ed.<\/p>\n\n\n\n<p>Plazma je tekut\u00e1 slo\u017eka krve, kter\u00e1 tvo\u0159\u00ed asi 55 % jej\u00edho objemu. Je p\u0159ev\u00e1\u017en\u011b tvo\u0159ena vodou (p\u0159ibli\u017en\u011b 90 %) a obsahuje d\u016fle\u017eit\u00e9 l\u00e1tky jako b\u00edlkoviny, elektrolyty, hormony a \u017eiviny. Plazma hraje kl\u00ed\u010dovou roli p\u0159i udr\u017eov\u00e1n\u00ed krevn\u00edho tlaku, regulaci t\u011blesn\u00e9 teploty a transportu l\u00e1tek po t\u011ble. Tuto \u017eivotn\u011b d\u016fle\u017eitou tekutinu mohou poskytnout pouze d\u00e1rci plazmy, nelze ji synteticky vytvo\u0159it ani vyrobit v laborato\u0159i. Pou\u017e\u00edv\u00e1 se k v\u00fdrob\u011b \u017eivot zachra\u0148uj\u00edc\u00edch l\u00e9k\u016f, jako je albumin, imunoglobuliny a faktory sr\u00e1\u017een\u00ed krve. D\u00e1rcovstv\u00ed plazmy pom\u00e1h\u00e1 zachra\u0148ovat a zlep\u0161ovat \u017eivoty lid\u00ed na cel\u00e9m sv\u011bt\u011b, kte\u0159\u00ed trp\u00ed \u017eivot ohro\u017euj\u00edc\u00edmi stavy, jako je hemofilie a dal\u0161\u00ed krv\u00e1civ\u00e9 poruchy, imunitn\u00ed nedostate\u010dnost, autoimunitn\u00ed onemocn\u011bn\u00ed a z\u00e1va\u017en\u00e9 krv\u00e1civ\u00e9 poruchy. Hraje tak\u00e9 kl\u00ed\u010dovou roli p\u0159i l\u00e9\u010db\u011b pacient\u016f s pop\u00e1leninami, t\u011b\u017ek\u00fdmi \u00farazy nebo b\u011bhem operac\u00ed, kdy je nezbytn\u00e9 doplnit objem krve a zajistit stabilitu krevn\u00edho ob\u011bhu. Nav\u00edc je nezbytn\u00e1 pro v\u00fdrobu n\u011bkter\u00fdch vakc\u00edn a p\u0159i terapii n\u00e1dorov\u00fdch onemocn\u011bn\u00ed. D\u00edky d\u00e1rc\u016fm plazmy mohou zdravotnick\u00e9 syst\u00e9my po cel\u00e9m sv\u011bt\u011b poskytovat kritickou p\u00e9\u010di t\u011bm, kte\u0159\u00ed ji nejv\u00edce pot\u0159ebuj\u00ed. Po odb\u011bru se mus\u00ed co nejrychleji zmrazit, aby si zachovala sv\u00e9 jedine\u010dn\u00e9 vlastnosti. Zamra\u017een\u00e9 vydr\u017e\u00ed mnohem d\u00e9le \u2013 i n\u011bkolik m\u011bs\u00edc\u016f. P\u0159i manipulaci s ml\u00e9kem je ov\u0161em d\u016fle\u017eit\u00e1 zv\u00fd\u0161en\u00e1 hygiena, steriln\u00ed ods\u00e1va\u010dka i n\u00e1doby, do kter\u00fdch se ml\u00e9ko odkl\u00e1d\u00e1. Ml\u00e9ko se doporu\u010duje skladovat rad\u011bji v mal\u00fdch d\u00e1vk\u00e1ch (60-120 ml), aby se p\u0159ede\u0161lo zbyte\u010dn\u00e9mu pl\u00fdtv\u00e1n\u00ed. Zmrazen\u00ed t\u011bchto materi\u00e1l\u016f je prov\u00e1d\u011bno za pou\u017eit\u00ed kryoprotektivn\u00edch l\u00e1tek a velmi n\u00edzk\u00fdch teplot (obvykle -196 \u00b0C) v kapaln\u00e9m dus\u00edku, aby se zabr\u00e1nilo po\u0161kozen\u00ed bun\u011bk a tk\u00e1n\u00ed.
