Ta krom\u011b onemocn\u011bn\u00ed srdce a c\u00e9v a \u0159ady dal\u0161\u00edch komplikac\u00ed zp\u016fsobuje tak\u00e9 probl\u00e9my s ot\u011bhotn\u011bn\u00edm. Po\u010det \u017een s&nbsp;obezitou p\u0159itom za\u010d\u00edn\u00e1 strm\u011b stoupat pr\u00e1v\u011b ve v\u011bku, kdy uva\u017euj\u00ed o zalo\u017een\u00ed rodiny. Podle odborn\u00edk\u016f se ka\u017ed\u00fdm shozen\u00fdm kilem \u0161ance na po\u010det\u00ed d\u00edt\u011bte zvy\u0161uje \u2013 kl\u00ed\u010dov\u00e1 je v\u0161ak dlouhodob\u00e1 zm\u011bna \u017eivotn\u00edho stylu. O vlivech obezity na \u017eensk\u00e9 reproduk\u010dn\u00ed zdrav\u00ed dnes debatovali odborn\u00edci v\u202fr\u00e1mci odborn\u00e9ho semin\u00e1\u0159e na p\u016fd\u011b Poslaneck\u00e9 sn\u011bmovny P\u010cR.&nbsp;<\/strong><\/p>\n\n\n\n<p>\u201e<em>V dob\u011b, kdy se v tuzemsku pot\u00fdk\u00e1me s neust\u00e1le klesaj\u00edc\u00ed porodnost\u00ed, p\u0159edstavuje toto dal\u0161\u00ed z\u00e1sadn\u00ed oblast, na kterou je t\u0159eba se zam\u011b\u0159it, aby k\u0159ivka poklesu po\u010dtu narozen\u00fdch d\u011bt\u00ed nebyla v budoucnu je\u0161t\u011b strm\u011bj\u0161\u00ed,\u201c <\/em>\u0159\u00edk\u00e1 m\u00edstop\u0159edseda V\u00fdboru pro zdravotnictv\u00ed Poslaneck\u00e9 sn\u011bmovny PharmDr. Petr Fifka (ODS), organiz\u00e1tor a moder\u00e1tor kulat\u00e9ho stolu s&nbsp;n\u00e1zvem Vliv obezity a \u017eivotn\u00edho stylu na plodnost.<\/p>\n\n\n\n<p>Podle dat \u00dastavu zdravotnick\u00fdch informac\u00ed a statistiky (\u00daZIS) m\u00e1 zdravou v\u00e1hu 59 % obyvatel ve v\u011bku 16 a\u017e 34 let, od 35 let v\u00fd\u0161e u\u017e ale tento pod\u00edl kles\u00e1 na pouh\u00fdch 37 %. Odborn\u00edci na l\u00e9\u010dbu neplodnosti se shoduj\u00ed, \u017ee obezita m\u00e1 negativn\u00ed vliv na schopnost po\u010d\u00edt d\u00edt\u011b. Nadbytek tuku zp\u016fsobuje v&nbsp;\u017eensk\u00e9m t\u011ble hormon\u00e1ln\u00ed nerovnov\u00e1hu, ztr\u00e1tu ovulace a nepravideln\u00fd menstrua\u010dn\u00ed cyklus, \u010d\u00edm\u017e br\u00e1n\u00ed po\u010det\u00ed. Obezita tak\u00e9 komplikuje n\u00e1slednou l\u00e9\u010dbu neplodnosti. <em>\u201eU pacientek s&nbsp;BMI nad 30\u201335, co\u017e ozna\u010dujeme jako obezitu prvn\u00edho stupn\u011b, v\u00fdrazn\u011b kles\u00e1 \u00fasp\u011b\u0161nost asistovan\u00e9 reprodukce. Hormon\u00e1ln\u00ed injekce maj\u00ed ni\u017e\u0161\u00ed \u00fa\u010dinek a hroz\u00ed komplikace p\u0159i anestezii i samotn\u00e9m odb\u011bru vaj\u00ed\u010dek,\u201c<\/em> \u0159\u00edk\u00e1 MUDr. \u0160t\u011bp\u00e1n Macha\u010d, p\u0159edseda Sekce asistovan\u00e9 reprodukce \u010cesk\u00e9 gynekologicko-porodnick\u00e9 spole\u010dnosti \u010cLS JEP, a dod\u00e1v\u00e1: <em>\u201eP\u0159itom u n\u011bkter\u00fdch pacientek s&nbsp;polycystick\u00fdmi vaje\u010dn\u00edky m\u016f\u017ee u\u017e 5% pokles hmotnosti znovu nastartovat ovulaci.\u201c<\/em><\/p>\n\n\n\n<p>\u017deny, kter\u00fdm se neda\u0159\u00ed p\u0159irozen\u011b po\u010d\u00edt d\u00edt\u011b, bojuj\u00ed tak\u00e9 s&nbsp;\u010dasem. Zejm\u00e9na po 35. roku v\u011bku za\u010d\u00edn\u00e1 skokov\u011b klesat kvalita vaj\u00ed\u010dek a \u0161ance na ot\u011bhotn\u011bn\u00ed se zmen\u0161uj\u00ed. Odborn\u00edci v\u0161ak z\u00e1rove\u0148 d\u016frazn\u011b varuj\u00ed p\u0159ed p\u0159ekotn\u00fdm hubnut\u00edm, kdy \u017eena rychle ztr\u00e1c\u00ed nadbyte\u010dn\u00e1 kila t\u00edm, \u017ee se dostane do tzv. stavu ket\u00f3zy. <em>\u201eExtr\u00e9mn\u00ed diety l\u00e9\u010dbu \u010dasto je\u0161t\u011b zhor\u0161\u00ed, proto\u017ee je t\u011blo ve stresu. Doch\u00e1z\u00ed k&nbsp;hormon\u00e1ln\u00ed dysbalanci a naru\u0161en\u00ed menstrua\u010dn\u00edho cyklu. Pacientk\u00e1m doporu\u010duji hubnout pod dohledem odborn\u00edk\u016f, tedy l\u00e9ka\u0159e, nutri\u010dn\u00edho terapeuta a fyzioterapeuta \u010di tren\u00e9ra,\u201c<\/em> vysv\u011btluje specialista na l\u00e9\u010dbu neplodnosti MUDr. Marcel \u0160telcl,&nbsp;vedouc\u00ed l\u00e9ka\u0159 reproduk\u010dn\u00ed kliniky ReproGenesis.<\/p>\n\n\n\n<p>Pr\u00e1v\u011b dlouhodob\u00e1 a komplexn\u00ed l\u00e9\u010dba je cestou, jak obezitu porazit. Nejde jen o \u0161patn\u00fd \u017eivotn\u00ed styl nebo nedostatek v\u016fle, ale o nemoc, kterou trp\u00ed v\u00edce ne\u017e t\u0159etina \u010desk\u00fdch \u017een. Rizikov\u00fdm pacientk\u00e1m s\u202fobezitou, kter\u00e9 nemohou ot\u011bhotn\u011bt, m\u016f\u017ee pomoci terapie modern\u00edmi l\u00e9ky. <em>\u201eT\u00e9m\u011b\u0159 v\u0161ichni pacienti s obezitou maj\u00ed za sebou opakovan\u00e9 snahy zhubnout a zm\u011bnit \u017eivotn\u00ed styl, ale boj s&nbsp;vlastn\u00edm t\u011blem prohr\u00e1vaj\u00ed. Obezita je nemoc, s n\u00ed\u017e je nutn\u00e9 bojovat v\u0161emi prost\u0159edky, kter\u00e9 m\u00e1me k&nbsp;dispozici, v\u010detn\u011b farmakoterapie. Ta, kterou m\u00e1me v&nbsp;sou\u010dasn\u00e9 dob\u011b k&nbsp;dispozici, m\u016f\u017ee d\u00edky sv\u00e9 \u00fa\u010dinnosti pacient\u016fm se sni\u017eov\u00e1n\u00edm hmotnosti v\u00fdrazn\u011b pomoci,\u201c<\/em> \u0159\u00edk\u00e1 prof. MUDr. Martin Haluz\u00edk, p\u0159edseda \u010cesk\u00e9 obezitologick\u00e9 spole\u010dnosti \u010cLS JEP.<\/p>\n\n\n\n<p>Za rozvojem obezity zpravidla stoj\u00ed hned n\u011bkolik faktor\u016f. Roli hraje krom\u011b nespr\u00e1vn\u00fdch stravovac\u00edch n\u00e1vyk\u016f i psychika, genetick\u00e1 predispozice, jin\u00e9 onemocn\u011bn\u00ed, ale nap\u0159\u00edklad tak\u00e9 \u0161patn\u00e9 vzorce p\u0159ej\u00edman\u00e9 z&nbsp;d\u011btstv\u00ed. <em>\u201e\u010cast\u00fdm m\u00fdtem je, \u017ee se \u017een\u00e1m s&nbsp;p\u0159ib\u00fdvaj\u00edc\u00edm v\u011bkem zpomaluje metabolismus, ale nen\u00ed tomu tak. V\u00edme, \u017ee mezi 20 a\u017e 60 lety je naopak velmi stabiln\u00ed. Probl\u00e9m je sp\u00ed\u0161e v&nbsp;\u017eivotn\u00edm stylu, sedav\u00e9m zp\u016fsobu zam\u011bstn\u00e1n\u00ed a v&nbsp;nedostatku pohybu. V&nbsp;minulosti jsme museli vynalo\u017eit fyzick\u00e9 \u00fasil\u00ed, abychom si zajistili potravu, dnes si j\u00eddlo objedn\u00e1me online. A m\u00edsto kvalitn\u00edch surovin \u010dasto vol\u00edme sp\u00ed\u0161e pr\u016fmyslov\u011b zpracovan\u00e9 potraviny, kter\u00e9 obsahuj\u00ed velk\u00e9 mno\u017estv\u00ed kalori\u00ed, ale nezasyt\u00ed n\u00e1s,\u201c<\/em> popisuje nutri\u010dn\u00ed terapeut Bc. Miloslav \u0160indel\u00e1\u0159.<\/p>\n\n\n\n<p>P\u0159esto\u017ee je v&nbsp;boji s&nbsp;obezitou d\u016fle\u017eit\u00e9 tak\u00e9 sportovat, nevhodn\u00fd j\u00eddeln\u00ed\u010dek kompenzovat pohybem nelze \u2013 z\u00e1sadn\u00ed je pr\u00e1v\u011b v\u00fd\u017eiva. U \u017een m\u016f\u017ee hubnut\u00ed komplikovat p\u0159irozen\u00e9 kol\u00eds\u00e1n\u00ed hormon\u016f b\u011bhem menstrua\u010dn\u00edho cyklu. <em>\u201eHormony nejsou p\u0159\u00edmou p\u0159\u00ed\u010dinou obezity, ale ovliv\u0148uj\u00ed p\u0159\u00edjem a v\u00fddej energie. Plat\u00ed to samoz\u0159ejm\u011b i pro mu\u017ee. Pokud jsme v&nbsp;chronick\u00e9m stresu a p\u0159ej\u00edd\u00e1me se, tak nutn\u011b p\u0159ibereme,\u201c<\/em> dod\u00e1v\u00e1 \u0160indel\u00e1\u0159 s&nbsp;t\u00edm, \u017ee je podstatn\u00e9, aby \u017eeny nejen zhubly, ale zdravou v\u00e1hu si tak\u00e9 udr\u017eely.<\/p>\n\n\n\n<p>Pr\u00e1v\u011b dlouhodob\u00e9 zdrav\u00ed \u017een \u0159e\u0161\u00ed tak\u00e9 l\u00e9ka\u0159i v&nbsp;l\u00e9\u010db\u011b neplodnosti. Jejich c\u00edlem toti\u017e nen\u00ed pouze to, aby \u017eena ot\u011bhotn\u011bla, ale tak\u00e9 aby porodila zdrav\u00e9 d\u00edt\u011b, o kter\u00e9 zvl\u00e1dne plnohodnotn\u011b pe\u010dovat. Ob\u00e9zn\u00ed \u017eeny maj\u00ed v t\u011bhotenstv\u00ed probl\u00e9my s&nbsp;diabetem a vysok\u00fdm krevn\u00edm tlakem, nav\u00edc maj\u00ed a\u017e o&nbsp;t\u0159etinu vy\u0161\u0161\u00ed riziko nedono\u0161en\u00ed d\u00edt\u011bte.<\/p>\n\n\n\n<p> <em>\u201eOb\u00e9zn\u00ed pacientky rod\u00ed v\u011bt\u0161\u00ed d\u011bti, \u010dasto c\u00edsa\u0159sk\u00fdm \u0159ezem, kter\u00fd s&nbsp;sebou nese rizika pro matku i d\u00edt\u011b. Nav\u00edc se u \u017een s vy\u0161\u0161\u00ed hmotnost\u00ed h\u016f\u0159e hoj\u00ed opera\u010dn\u00ed r\u00e1ny. Nekon\u010d\u00ed to ale ani porodem. V\u00edme, \u017ee predispozice k&nbsp;obezit\u011b se p\u0159en\u00e1\u0161\u00ed z&nbsp;matky na d\u00edt\u011b,\u201c<\/em> \u0159\u00edk\u00e1 MUDr.&nbsp;Macha\u010d.<\/p>\n\n\n\n<p><strong>Vliv obezity a \u017eivotn\u00edho stylu na plodnost<\/strong><br \/><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Porodnost v \u010cesk\u00e9 republice<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>V roce\u00a0<strong>2023<\/strong>\u00a0se \u017eiv\u011b narodilo\u00a0<strong>91,1 tis\u00edce d\u011bt\u00ed<\/strong>, co\u017e bylo\u00a0<strong>o 10,2 tis\u00edc m\u00e9n\u011b ne\u017e v roce 2022<\/strong>.<\/li>\n\n\n\n<li>V roce\u00a0<strong>2024<\/strong>\u00a0se \u017eiv\u011b narodilo\u00a0<strong>84,3 tis\u00edce d\u011bt\u00ed<\/strong>, tedy o\u00a0<strong>dal\u0161\u00edch 6,8 tis\u00edc m\u00e9n\u011b ne\u017e v roce 2023<\/strong>.<\/li>\n\n\n\n<li>Jde o\u00a0<strong>dva v\u00fdrazn\u00e9 meziro\u010dn\u00ed poklesy v \u0159ad\u011b<\/strong>, s celkov\u00fdm \u00fabytkem t\u00e9m\u011b\u0159 17 tis\u00edc narozen\u00fdch b\u011bhem dvou let.<\/li>\n\n\n\n<li>\u00dahrnn\u00e1 plodnost v roce\u00a0<strong>2023 klesla na 1,45 d\u00edt\u011bte na \u017eenu<\/strong>.<\/li>\n\n\n\n<li>Pr\u016fm\u011brn\u00fd v\u011bk \u017eeny p\u0159i narozen\u00ed\u00a0<strong>prvn\u00edho d\u00edt\u011bte vzrostl za menstrua\u010dn\u00edho cyklu.<\/li>\n\n\n\n<li>Vy\u0161\u0161\u00ed mno\u017estv\u00ed b\u0159i\u0161n\u00edho tuku zvy\u0161uje riziko\u00a0<strong>hormon\u00e1ln\u00edch poruch<\/strong>\u00a0spojen\u00fdch s plodnost\u00ed.<\/li>\n\n\n\n<li>\u017deny s obezitou maj\u00ed vy\u0161\u0161\u00ed v\u00fdskyt\u00a0<strong>inzul\u00ednov\u00e9 rezistence<\/strong>\u00a0a ni\u017e\u0161\u00ed hladinu\u00a0proteinu <strong>SHBG<\/strong>, kter\u00fd ovliv\u0148uje pohlavn\u00ed hormony \u2013 androgeny a estrogeny.<\/li>\n<\/ul>\n\n\n\n<p><strong>Nepravideln\u00fd cyklus<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Vy\u0161\u0161\u00ed BMI je spojeno s\u00a0<strong>rizikem nepravideln\u00fdch menstrua\u010dn\u00edch
